Critical: WHEN TO WASH Observation: Employee observed wearing gloves when handling raw hamburger patty. Gloves were removed. Employee moved to working with the ready-to-eat hamburger before washing hands.
Correction: Before working with food, clean equipment, utensils, and single-use or single service articles food employees shall properly wash their hands. Even though gloves are being worn, a proper hand wash needs to be completed before working with the ready-to-eat food. An option may be to use tongs instead of handling the raw hamburger patty. Critical: SEWAGE - BACKFLOW PREVENTION Observation: The ice machine currently has an air break instead of a proper air gap. Hoses from the ice machine are going in to the sewage pipe.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain. Remove the hoses from the pipe and make a proper air gap. Correct By: 20-Jan-2015
EQUIPMENT AND UTENSILS - FOOD CONTACT SURFACES, CLEANABILITY Observation: Food storage containers located in kitchen area have noticable cracks and broken pieces.

THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE Observation: The Frigidaire freezer upstairs, three GE chest freezers in the basement, two door chest cooler in the bar area, and Perlick cooler in the bar are not equipped with an integral or permanently affixed temperature measuring device.
Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Correct By: 20-Jan-2015
NON-FOOD CONTACT SURFACES - CLEANING FREQUENCY Observation: The freezer door handles found upstairs and in the basement and the Perlick cooler are not cleaned on a frequent basis. The Perlick cooler has noticable residue on the bottom.
