Correction: Repair/replace the door seal so it is smooth and intact.

Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER Observation: There are 2 pans of raw chicken breasts stored over corned beef and steaks in the True 2 door cooler. There is raw fish stored over vegetables salsa, tortilla chips, sauces in the Traulsen cooler.
Correction: Prevent nesting of raw animal foods and store raw animal foods in order of descending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. Store ready-to-eat foods above raw animal foods. Correct By: 19-Mar-2015
Critical: DATE MARKING - DISPOSITION (repeated violation) Observation: Beer cheese dip and salsa in the 3 door prep cooler and milk in the bar cooler has exceeded its date mark and spinach dip in the Maximum cooler is not provided with a date mark. foods with expired date marks were discarded during the inspection.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked. Correct By: 19-Mar-2015

Critical: DATE MARKING - DISPOSITION (corrected on site) Observation: Black bean & corn relish is date marked 2/20. Hard boiled eggs are date marked 2/22. Goat cheese is date marked 1/29. All were discarded during the inspection.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.
Critical: FOOD CONTACT SURFACES - WAREWASHING EQUIPMENT, CHEMICAL, TEMPERATURE, Ph, CONCENTRATION AND HARDNESS (corrected on site) Observation: The chlorine sanitizer on the bar glass dish machine is at 0 ppm. Repair was done during the inspection and is now 50ppm.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.
THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE Observation: No thermometer was located in the True 3 door under counter cooler in the basement prep area. The thermometer in the Norlake 2 door under counter cooler in the bar is broken.
Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device.
INSECT CONTROL DEVICES, DESIGN AND INSTALLATION - IMPROPERLY LOCATED Observation: The insect control device is located over an open box of potatoes.
Correction: Remove or relocate this device so that it is not over any food or food prep area.
INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS Observation: The exterior door in the basement is not self-closing or tight-fitting.
Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.
