  • Critical: PREVENTING CONTAMINATION FROM HANDS - NO BARE HAND CONTACT WITH RTE-FOODS (corrected on site)
    Observation: Employee observed handling ready-to-eat food (lettuce, tomatoes) with bare hands. Gloves were then worn. Will provide information about creating a written bare hand contact plan.
    Correction: Employees shall use single-use gloves, utensils, bakery papers or other approved means when handling ready-to-eat foods.
  • Critical: PHF/TCS, COLD HOLDING
    Observation: Bacon, marinara, tomato relish and pico in the top of the oven prep cooler is cold held at 44-49°F. The cooler was turned colder during the inspection. The fish on ice closest to the fryer is 60°. Ice bath adjusted during inspection.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. If foods are kept on ice, the level of ice must be to the level of the food. Correct By: 27-Jan-2016
  • EQUIPMENT AND UTENSILS - PROPER ADJUSTMENT
    Observation: The top of the left door seal of the pizza prep cooler is torn.
    Correction: Repair/replace the torn door seal.

    Observation: A quat test kit is not available for checking sanitizer concentrations upstairs.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
  • FOOD CONTACT SURFACES - SOILED
    Observation: Meat slicer is visibly soiled.
    Correction: Maintain food contact surfaces in a clean condition.
  • HAND DRYING PROVISION
    Observation: No single-use toweling or other hand drying device available at handwashing sink upstairs for hand drying.
    Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR (repeated violation)
    Observation: Multiple boxes of food were found to be stored on the floor in the walk-in cooler and walk-in freezer. Storage compacity in the walk-in freezer is not sufficient to keep food off of the floor.
    Correction: Store all food items 6 inches above the floor.
  • EQUIPMENT AND UTENSILS - GOOD REPAIR
    Observation: Ice scoop in bar is cracked by the handle.
    Correction: Repair equipment to good condition or remove from premise.
  • FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
    Observation: The coving on the front of the walk-in freezer is peeling away.
    Correction: Provide base coving.
  • LIGHT BULBS - PROTECTIVE SHIELDING
    Observation: Lights located in the upstairs storage room are not shatter resistant.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.

  • Critical: COOLING (corrected on site)
    Observation: Container of clam chowder in the Arctic Air single door cooler did not cool properly from last night and is at 46-52°F after more than 6 hours. The soup was discarded during the inspection. The cooling procedure was explained to inspector and is acceptable. An ice wand is used to cool then it is normally put in the walk-in cooler to completely cool, but it was put in the single door cooler in the prep area last night. The normal procedure will be followed and temperature will be monitored.
    Correction: Adjust method or procedures to cool foods from 135°F to 70°F in the first 2-hours and from 70°F to 41°F in an additional 4-hours or if prepared or received at ambient temperatures cool to 41°F in 4-hours.
  • Critical: DATE MARKING - DISPOSITION (corrected on site)
    Observation: There was a container of shredded chicken in the Aquafina cooler with a date mark of 12/2 and a container of chicken salad in the Arctic Air 2 door cooler in the prep area with a date mark of 12/5. Both items were discarded during the inspection. Other date marks were within 7 days.
    Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.
  • Critical: PLUMBING - BACKFLOW PREVENTION - AIR GAP
    Observation: No air gap provided on the downstairs ice machine.
    Correction: Provide an air gap on water supply side to protect water supply.
  • Critical: TOXIC SUBSTANCES - STORAGE
    Observation: Multiple containers of chafing fuel are stored above sugar packets and chocolate syrup in the dry storage area.
    Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: Boxes of food are stored on the floor in the walk-in cooler and walk-in freezer.
    Correction: Store all food items 6 inches above the floor.
  • THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
    Observation: There was no thermometer located in the Aquafina glass door cooler, True 2 top prep cooler, or the True 2 glass door cooler in the side bar.
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device.
  • FOOD CONTACT SURFACES - MICROWAVE CLEANED
    Observation: Inside of microwave is soiled.
    Correction: Clean and maintain microwave.
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
    Observation: Boxes of to-go clam shell containers and cups are improperly stored on the floor in the upstairs dry storage area.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and at least 6" off the floor.
