Observation: No date marking observed throughout establishment. Ready to eat food that was not date marked included coleslaw, lasagna meatballs, and cooked pasta.

Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.

Observation: Wooden shelf in the back area of the kitchen are sagging and separating.

Observation: The Kitchen Pizza Prep Cooler, Kitchen Delfield Prep Cooler and Kitchen Ascend Cooler were leaking fluid (puddling water in bottom of units).
Correction: Repair equipment to good condition or remove from premise.

Observation: Cutting boards located in the kitchen are deeply grooved.
Correction: Resurface or discard cutting boards or blocks with excessive scratching or scoring.

Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
- INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS
Observation: The exterior door in the kitchen is not tight-fitting. A gap was observed between the bottom of the door and threshold.
Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.

Observation: The floor under and behind equipment was soiled.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.
