- Critical: SEWAGE - BACKFLOW PREVENTION (repeated violation)
Observation: An air break exists between the sewage system and a drain from the ice bin.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Provide an air gap on the ice bin drain. Correct By: 09-Jul-2015
- FOOD TEMPERATURE MEASURING DEVICES - SMALL DIAMETER PROBE
Observation: No thermometer is present for monitoring temperatures in thin mass foods or cold held foods.
Correction: Provide a thermometer capable of taking temperatures in thin mass food items and cold held foods. Correct By: 20-Jul-2015
- THERMOMETERS - FUNCTIONALITY - WARMEST PART OF UNIT
Observation: Thermometers in the True GDM-23 one door cooler and True GDM-37 two door cooler are not located in the warmest part of each unit.
Correction: Reposition thermometers or add thermometers so that there are thermometers located in the warmest part each refrigeration unit. Correct By: 06-Aug-2015
- EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS
Observation: DG4A gyro machine is not ANSI certified or approved by the department.
Correction: Unapproved equipment shall be removed from food service. Discussed comparable compliance application.
- FOOD CONTACT SURFACES - NON TCS/PHF -IMPROPER CLEANING
Observation: Ice machine has excessive biofilm inside the machine and at the end of the ice melt drain line.
Correction: Clean the ice machine within 24 hours to remove the biofilm and clean as often as necessary to prevent the accumulation of biofilm.
- PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
Observation: The hot water control on the men's room faucet is stuck open, and the supply valve is shut off. Turning on the supply valve provides hot water, but cannot be shut off at the faucet.
Correction: The plumbing system shall be maintained in good repair. Repair or replace faucet in men's room. Correct By: 20-Jul-2015
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: The walls and ceiling of the upstairs walk-in cooler are wood chip-board which is not smooth nor easily cleanable.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable. Correct By: 06-Oct-2015
- TOILET FACILITIES - ENCLOSED (repeated violation)
Observation: Men's and women's toilet room doors are not self-closing. There is evidence of a painted over hole in the doors which may have been a mount location for a self-closing door mechanism at one time.
Correction: Provide self-closing door mechanisms on the men's and women's toilet room doors. Correct By: 06-Aug-2015
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED (repeated violation)
Observation: There is a gap in the rear exterior door to the bar which could allow the entrance of insects or rodents.
Correction: Repair this gap so that entrance by insects or rodents into the food establishment is prevented. Correct By: 06-Aug-2015

- Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
Observation: Ham and sliced onions in True TWT-72 3-door cooler are not date marked.
Correction: Food was date marked during the inspection as required. Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. The growth of some bacteria, such as Listeria monocytogenes, is
significantly slowed but not stopped by refrigeration. Over a period of time, this and similar organisms may increase their risk to public health in ready-to-eat foods.
- Critical: SEWAGE - BACKFLOW PREVENTION
Observation: Air breaks exist between the sewage system and a drains from the ice machine and ice bin. The drain pipes extend down into the receptors or are directly connected.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connections. Provide air gaps on the drains from the ice machine and ice bin. Correct By: 21-Mar-2014
- KITCHENWARE AND TABLEWARE - HANDLED TO PREVENT CONTAMINATION
Observation: It was observed that eating utensils are stored or displayed in plastic drawers where the food or lip contact surface could be contaminated.
Correction: Store utensils in a manner to avoid contamination to the food and lip contact surface. Correct By: 15-Mar-2014
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The floor, walls, and ceiling in the walk-in cooler is not smooth nor easily cleanable.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable. Correct By: 14-Mar-2015
- TOILET FACILITIES - ENCLOSED
Observation: Toilet room doors for men and women are not self-closing.
Correction: Provide self-closing door mechanisms on bathroom doors. Correct By: 28-Mar-2014
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: There is a hole or gap in the rear entry door which could allow the entrance of insects or rodents.
Correction: Repair this hole or gap so that entrance by insects or rodents into the food establishment is prevented. Correct By: 28-Mar-2014
