    Observation: Partially cooked chicken is cold held at 65°F on food preparation counter by the fried rice. Operator stated chicken was partially cooked yesterday.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Prior written approval from the Department and labeling of food is requried before use of time as a public health control. Operator returned chicken to the walk-in cooler. Discard chicken within 4 hours of removal from the walk-in cooler.
  • NON-CONTINUOUS COOKING OF RAW ANIMAL FOODS - WRITTEN PLAN
    Observation: The non-continuous cook process for chicken has no prior approval or the approved plan is not being followed. Operator stated chicken on counter was previously cooked to 155F.
    Correction: Food improperly prepared shall be discarded. Submit plan for approval by regulatory authority and once approved shall be follwed by management and employees. Correct By: 11-Oct-2015
  • FOOD CONTAMINATION PREVENTED - SEGREGATION AND LOCATION OF DISTRESSED MERCHANDISE (corrected on site)
    Observation: Can of bamboo shoots dented along the seam is stored next to other food items intended for use.
    Correction: Store all distressed food items in designated areas which are separate from other foods. Operator moved can of bamboo shoots to the rear of the kitchen.

    Observation: Employees noted wearing wristwatch and bracelet while working with food.
    Correction: Except for a plain ring such as a wedding band, prohibit food employees wearing jewelry on the hands or wrists while working with food. Correct By: 27-Sep-2015

    Observation: Bulk food containers of flour, starch, and sugar are not labeled.
    Correction: Bulk food label or placard information shall include: common name of food, ingredients in order of predominance, and food allergens.
  • SINGLE-USE AND SINGLE SERVICE ARTICLES - USE LIMITATIONS
    Observation: Clear, blue, and red plastic bags hanging over the three compartment sink and handwashing sink were observed being reused. These plastic bags originate from the food supplier to store raw animal food and other food. Observed food in the freezer stored in these bags which did not originally come in the bag.
    Correction: Discard single-use and single service articles after initial use. Operator discarded bags hanging over handwashing sink and three compartment sink. All food remaining in the freezer which is stored in reused bags must be examined closely for contamination from pieces of plastic. If pieces of plastic is frozen into the food or found in the food, then the food must be discarded. If after examining the food closely no plastic pieces are found, the food must all be cooked to 165F before service. Correct By: 27-Dec-2015

    Observation: Fried chicken and cooked rice from walk-in cooler is held on counter, is at 46°F and 43.5F, and is climbing to ambient temperatures..
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Chicken and rice returned to the walk-in cooler during inspection. Discard any foods which have been held above 41F for more than four hours or for an unknown amount of time.
  • THERMOMETERS - AMBIENT AIR AND WATER - ACCURACY
    Observation: Thermometer in prep cooler measuring ambient air is not accurate to ± 3°F. Thermometer shows 19F, but ambient air and food inside is 38F.
    Correction: Replace thermometer. Correct By: 25-Apr-2014
  • TOXIC SUBSTANCES - SANITIZER CRITERIA
    Observation: Scented (lemon) bleach used in the food establishment is not approved.
    Correction: Sanitizer shall meet the requirements under 40 CFR 180.940. Replace bleach today with a type that is non-scented and bears an EPA registration number. Correct By: 18-Apr-2014

    Observation: Rear screen door has holes and gaps in the screening. Operator has some rolls of material for repair.
    Correction: Doors and windows kept open to provide ventilation shall be provided with screens to prevent entrance of rodents and pests. Repair or replace screening on the screen door.. Correct By: 25-Apr-2014
  • CLEANING, FREQUENCY AND RESTRICTIONS
    Observation: The walls of the walk-in cooler have black mold growing from apparent food spills.
    Correction: Clean the physical facilities at a frequency necessary to keep them clean. Remove mold from walls within 24 hours. Clean all floors, walls, and ceiling of walk-in cooler with warm water and soap by next Wednesday, 4/23.
