Observation: The consumer advisory for undercooked proteins was on the menu, but was not written properly.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition. Guidance will be provided as to proper consumer advisory language.
- FOOD PREPARATION SINK (repeated violation)
Observation: The facility does not have a separate food preparation sink. The facility is currently using the air gapped sanitizer section of the 3 comparment sink.
Correction: A food preparation sink should be provided for significant food preparation or in instances where food preparation is occurring below the flood level rim of a sink compartment such as thawing or vegetable washing.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: The ceiling tiles in the waitress area are not smooth, washable and non absorbent. The floor in the following area must be replaced within 1 year - bar, waitress and storage room.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.

Observation: Bella Nesco unit is not ANSI certified or approved by the department.
Correction: Unapproved equipment shall be removed from food service.

Observation: The beer walkin refrigeration unit was leaking water onto the floor. The ice machine was not operating properly and had the access door removed for service. The sneeze shield on the buffet table is cracked and must be replaced.
Correction: Repair equipment to good condition or remove from premise.

Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
