Observation: The plumbing system is not properly designed, constructed or installed. A Y-splitter has been attached to the mop area faucet which may cause the backflow device to malfunction.
Correction: Design, construct, install, and maintain the plumbing system to conform to the State Uniform Plumbing Code. Remove splitter. Provide permanent-T connection plumbing for permanent plumbing use if more connections are needed.

Observation: A chlorine test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions. Recommended warewashing machines and wall dispenser be tested at least weekly if not daily. Correct By: 17-Apr-2015

Observation: Food pans are being stacked without being air-dried.
Correction: Air dry all clean utensils completely before storage and before contact with food. Correct By: 10-Apr-2015

Observation: It was observed that eating utensils are set at tables partially wrapped in napkins where the food or lip contact surface could be contaminated.

- Critical: SEWAGE - BACKFLOW PREVENTION
Observation: Air breaks exist between the sewage system and a drains from the server's area ice bin (integrated into the Coke dispenser) and the food preparation sink in the kitchen.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Provide full air gaps. The lowest point on the drain pipes from the equipment must be vertically higher than the highest point on the receptor below it. Correct By: 13-Mar-2015
- CONSUMER ADVISORY
Observation: The consumer advisory on menu has an asterisk next to the cheeseburger slider on the kid's menu and is missing one from the advisory at the bottom.
Correction: Remove asterisk from the kid's menu cheeseburger since highly susceptible populations may not be served undercooked animal food. Add asterisk to consumer advisory to link the regular menu hamburger to the advisory. Correct By: 10-Apr-2015

- FOOD CONTAMINATION - FOOD STORAGE PROHIBITED AREAS
Observation: Coke rack of boxes and carbonator installed in the basement underneath two wastewater pipes are subject to contamination.
Correction: Remove food items from this area to an area not subject to contamination. Alternatively, provide secondary shielding underneath wastewater pipes which covers at least the full length of the equipment and is sloped away from equpiment. Correct By: 13-Mar-2015
- EQUIPMENT AND UTENSILS - FIXED , SPACING OR SEALING - INSTALLATION
Observation: A backsplash on the Beverage Air under-counter freezer gas a large gap which is not easily cleanable.
Correction: Seal the backsplash to the freezer easy cleaning or install it such that it can be easily removed for cleaning. Correct By: 11-Mar-2015

Observation: Clean beverage containers and food pans are being stored or stacked without being air-dried.

- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: There are gaps in the corner of the exterior door at the end of a hallway and a gap at the bottom of the side exit door between the kitchen and servers' area which could allow the entrance of insects or rodents.
Correction: Repair these gaps so that entrance by insects or rodents into the food establishment is prevented. Correct By: 24-Mar-2015
