did not receive my yellow work search form for eb (extension, claim)

hi..i usually receive my eb yellow work search form the saturday after i claim. i didn't receive it..so i expected it today (monday)..it wasn't in the mail. i'm a little worried if it got lost in the mail..i need to mail it to them this friday. should i send my work searches written on a piece of paper friday? is that good enough or must it be the original work search sheet? i tried calling but i couldn't get through all morning

finally got through to an agent,.. she told me that if i didn't receive it by friday..that i should take a copy of the last work search form (that i scanned before i mailed it out) and white out the old info and put in new info

You actually have to do a work search form? I've always thought that was an empty threat, LOL... but I guess here in CA they just don't bother, unless maybe you're on the federal extension.

you should get it today. It was explained to me they send out the EB forms ont he day the payment is made or posted, usually the second business day. I claimed on Tuesday, they didnt send it out until Thursday, received on Saturday or something to that effect. they said all teh EB notices for peopel go out ont he same day. regardless of when you claim.
