Lots and lots of great recommendations here! All I can add is to suggest several extra crib sheets in basic colors (match the room theme if you know it). New moms always seem to register for the pretty matched set and maybe have a extra sheet on hand, but one night of mishaps and she's out of bedding. Babies R Us has basics that are inexpensive compared to the matchy-matchy sets. You can also stock up on sheets for the Pack 'n Play, bassinet, or whatever.

Waterproof lap pads are awesome as well. I like a big one under the crib sheet and smaller ones for changing pads. I also like disposable bibs and pads for the diaper bag.

Good suggestions Julia. We had a TON of those waterproof pads. We put them under the baby wherever he was (car seat, crib, bouncy seat, swing). Our favorite crib sheets were from Lands End.

I'm not sure if anyone has mentioned cloth diapers...

They are great to use as burping/spit up cloths - they are absorbent and large enough to really protect your clothes and/or provide coverage while nursing. Just launder as needed.

Later on you can use them as dust rags.

My go-to baby shower gift is a basket or canvas caddy (matching the room theme if i can tell from a registry) with the disposable pads (so great for bathroom changing areas with Lord-knows-what germs on them), "boogie wipes" for fun, an extra few sheets, and various pastes or washes. Oh, and one of those cute binkie pouches that attaches to a stroller handle or diaper bag strap. Those came out in the time between our daughters' and son's birth, and I remember wondering why nobody thought of it before.

The most useful item for my newborn was a boob.

But now looking back on it, the most helpful thing was a contoured changing pad I could put on my dresser to change her and not stoop over constantly, saved my back!

Muslin cloths - essential for a reflux baby! I always put one under her head when sleeping so I didn't have to change her sheets several times a day, just the muslin cloth
Battery powered baby monitor (rather than plug in) for outside naps

One little note on diapers. Diapers can be baby specific. My son would completely blow out of Huggies but Pampers would work. My DD could use anything.

If you like to be more practical, fill a basket (or a baby tub) with wipes, Desitin, liquid Tylenol, baby wash, and other baby items from your local drugstore. That stuff adds up!

The best item I got for my baby is Sleepy Wings swaddling jacket. He sleeps in it so much longer!!! I bought it on Amazon.
