For a number of reasons.

They ran out of money.
They squandered the money on individual social justice issues like free libraries, renovating people's homes, etc.
They lost focus..actually never had it because they chose not to have a leader.

I read the minutes of the NY crowd when they actually took and posted minutes.
All started off nice and political and turned social and infighting began and then they broke up.

OWS failed because the people bickering among themselves too much. Also, I think Americans are still too comfortable to truly want revolution.
