Raul Julio (Gomez on The Addams Family movies) looked thin in "The Burning Season", I
think he was in a few things later but in that movie he looked unwell.

Spencer Tracy was ill during the filming of "Guess Who's Coming to Dinner", and died a few days after the movie was completed.

Massimo Troisi had a heart condition and died hours after finishing filming of "Il Postino". To me, he appeared tired/ill in the movie--all the biking up & down those hills really did do him in.

That is sad about Massimo Traisi. He needed a stuntman!

John Ritter had a heart attack on the set of his show or dessing room? It must have been
hard for the cast to continue the show.

Child actress Heather O'Rourke was quite ill and puffy-faced (from the steroid prednisone) while filming Poltergeist III, and died a few months later at age 12.

I agree with two posts above about the person looked ill/not right in their last role: Heather O'Rourke and Jerry Orbach.

I would also add John Spencer from the West Wing. His appearance in the last season was significantly different than the first and each of those last episodes he looked more frail to me.

Jerome "Curly" Howard looked pretty rough in those last few Three Stooges shorts he made before he had the stroke that forced his retirement from the act.
