- Critical: Food employee(s) did not wash hands in situations that specifically require them to do so. Observed employee handling raw beef patty with gloves and not washing hands prior to putting on new pair of gloves.
employees shall wash hands as needed and change gloves as necessary to prevent cross contamination. Employees had discussed that washing hands prior to putting new gloves on is not a requirement all the time depending on situation. I had asked to review the employee SOP for handwashing and it was not available. Discussed proper procedures with employees.
- Critical: Raw shell eggs are not maintained at 45° F or less. PHF's (ex=slcied tomatoes, shredded lettuce, mayonnaise) were holding at 46F to 48F in the top of the prep line reach-in cooler. Several baskets of pre-cooked french fries were holding at 68F to 104F at room temperature.
To prevent the growth of pathogens, eggs that have not been treated to destroy all viable Salmonellae shall be stored refrigerated at an ambient air temperature of 45° F or less. Lid to cooler was closed. Manager had discussed that the fries shall be used within two hours once pre-cooked or voluntarily discarded. I had asked about the time stamp logs and the employees do not record times for time in lieu of temperature. Ensure french fries are discarded within the two hour limit. FACILITY SHALL PROVIDE APPROVED DOCUMENTATION OF TIME IN LIEU OF TEMPERATURE PROCESS

- The plumbing system was not properly maintained. Observed cold water only at women's restroom handwashing sink.
A plumbing system shall be properly maintained. Provide an adequate supply of hot running water at women's restroom handwashing sink.

- The physical facilities are not being maintained in good repair. Observed water leaking from the ceiling in the dining room. Per conversation with PIC, the air conditioner is causing the leak and maintenance has been contacted.
The physical facilities shall be maintained in good repair. Repair leak in ceiling to prevent water leakage into the dining area.
