Observation: An employee that was working with food was wearing a ring with a stone in it on one of her fingers.
Correction: The only ring that may be worn on the fingers, when working with food, is one plain band. No stones or rough surfaces are allowed. This is also true when disposable gloves are being worn.

Observation: The "person in charge," at the time of inspection, was not able to show us a signed and dated "employee health screening form" for each employee "on-duty," when we asked to see their signed and dated "employee health screening form.
Correction: We require that all employees who may work with food sign, and date, a copy of our "employee health screening form" (FDA model food code form 1-B). This form states that the employee understands that they are required to report to their supervisor if they are diagnosed with one of the big five illnesses associated with food establishments. These forms must be kept on file at the store, so that they are available for us to review on any inspection. This violation was immediately corrected by the establishment management.

Correction: The only ring that may be worn when working with food is one plain wedding band (no stones or rough surfaces). This is true, even when disposable gloves are worn.

- Critical: Food - Potentially Hazardous Food - Cold Holding*
Observation: Foods were "out of temperature" in the walk-in refrigerator. Chicken salad, individual servings, were at 47 F. Precooked chicken meat, diced, in a commercial plastic bag, was at 47 F. Onions slices were at 46 F.
Correction: When a potentially hazardous food goes out of temperature, that is above 41 F, it must be eaten, or discarded, within four hours of going out of temperature, or there is a risk of foodborne illness. The walk-in refrigerator needs to be serviced/maintained so that it is capable of holding all potentially hazardous foods at 41 F, or lower, at all times. A service call was immediately placed by the establishment management to have the walk-in refrigerator serviced.

Observation: There was a spray bottle of blue cleaner being stored above botttles of sorft drinks, on a storage rack, in the kitchen area.
Correction: Cleaners, sanitizers, and chemicals must always be stored below, and well separated from, amm foods and food contact surfaces. This violation was immediately corrected by the establishment management.

Observation: There were two soiled plastic bottles in the basin of the kitchen handwashing sink.
Correction: We require that handwashing sinks be used for only handwashing. Dirty "dishes" may not be stored in the kitchen handwashing sink, even temporarily. We need to do everything possible to encourage frequent, and when ever necessary, handwashing. This violation was immediately corrected by the establishment management.

Correction: The only ring that we allow to be worn on the hands, when preparing foods, is one plain wedding band. This violation was immediately corrected by the establishment managment.

Observation: There were two small knives being stored in the basin of the kitchen handwashing sink.
Correction: The handwashing sinks may only be used for handwashing. Nothing may be placed in the basin of the kitchen handwashing sink. We need to do everything possible to encourage frequent, and when ever necessary, handwashing. This violation was immediately corrected by the establishment management.
