Overall facility is visibly clean. Food items are typically order from other permitted locations and brought in. Personal drink was in refrigeration with food item served to community (corrected on the spot).Personal items must be stored separate or have a designated area to prevent cross contamination, Facility is required an annual water sample.

Observation: Single service items observed unprotected from contamination. Observed single serve cups stored on floor. Items must be stored 6 inches off floor to prevention contamination and harboring conditions for pest/rodents.
Correction: Store single service items in its original protective packaging or inverted in an approved dispenser.
- Handwashing Lavatory, Water Temperature, and Flow
Observation: When timed, the automatic shut off faucet at the handwashing sink locatedin the kitchen did not provide a flow of water for at least 15 seconds without the need to reactivate the faucet.
Correction: Adjust or replace the faucet to provide water tempered to at least 100°F and for at least 15 seconds to allow more effective handwashing and removal of fatty soils encountered in kitchens.

Observation: The refuse container used to store refuse/recyclables has no drain plug.Outside dumpster has no drain plug.
Correction: Replace the refuse container witha drain plug.

Observation: Observed methods are not being used to control pests, Observed several flies in kitchen and dining area. Recommend contacting certified pest control operator and have invoice faxed to the local Health Department in two weeks of this inspection date.
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

No food was at site during pre-opening inspection. Facility is Community Center owned by the City of Suffolk. Water is a community well water which required annual sampling and lab results sent to the Health Department. Discussed employee Health Policy and provided a copy of the FDA 1B form. Test strips fro the dishwasher and the 3 compartment sink are on order.no food was in the facility during the inspection. Two grease traps (3 compartment sink and prep sink) in facility and discussed grease handling with PIC. Air gaps are in the required locations and equipment are of commercial grade (NSF). Ensure food handler cards are obtained for all food workers prior to working with food. Submitted permit.

Observation: The 3 compartment sink is not sealed to adjoining equipment or walls.
Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.
