One food worker obtained food handler card from on-line course. This district does not accept on line testing of foodhandler cards. Please ensure this is corrected. Have plumbing leaks addressed before next inspection. Hood vents need cleaned. Establishment is to complete commissary agreement and forward to EH Office as soon as possible and commissary permit paperwork as required. Owners are to call EHS at next catering event (May) to inspect food processes.

Shelf inside RI designated for SVC items only. Employee health policy reviewed and in place. Reportable illness information as well as risk factors for illness also reviewed. Facility to be dually regulated by VDH and VDACS. PIC possesses current Suffolk health card. All City approval provided. Permit issued.
