| Inspection conducted prior to opening of facility. Facility is within substantial compliance with the Virginia Department of Health Food Regulations, therefore permit is issued. The flight-style dish machine is not meeting minimum requirements for cleaning and sanitizing equipment at this time. Representatives will be out tomorrow to evaluate the machine. Until the machine is operating properly, the downstairs dish machine and three compartment sink on this floor will be utilized. Ensure physical facilities and equipment are thoroughly cleaned and sanitized prior to operating.

| The following items will need to be addressed prior to issuance of a health department permit to operate: 1. Obtain a Certificate of Occupancy from the Building Official. 2. The mop sink will need to be sealed to the wall. 3. The flight dish machine will need to be operational. At this time, it is tripping the breaker while in use. 4. Water connection to the ware washing room will need to be turned on again. 5. A final cleaning and sanitizing of small wares will need to be done. 6. A final thorough, detailed cleaning of the physical facilities will need to be conducted. 7. Facility is able to bring in product and begin training staff, if also cleared to do so by the Fire Marshall and Building Official.
