    Observation: Unwrapped or uncovered food in the reach in and walk in refrigerator. EHS observed many foods uncovered in refrigeration units. Items uncovered include cut vegetables, sauces, chicken, beef, etc.
    Correction: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.

    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination. EHS observed raw chicken stored on top of avacodoes box inside walk in cooler.
    Correction: Separate types of raw animal foods from each other such as beef, fish, lamb pork, and poultry during storage, preparation, holding, and display by the following: (a) Using separate equipment for each type. (b) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. (c) Preparing each type of food at different times or in separate areas.

    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: EHS observed the prepared ready-to-eat (RTE) sauces, chicken and sushi in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Cooked rive in reach in refrigeration cold holding at improper temperatures. EHS observed temperature at 45F. PIC states was made the day before. Rice observed portioned and wrapped in plastic wrap. EHS recommends wrapping only once rice is cold (41F and below).
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria. PIC discarded item

    Observation: Clean tableware were observed stored with the food-contact surface facing upward.

    Observation: Employees observed working in the food service area without proper hair restraints. Three food workers in kitchen with no hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: The cutting board(s) along the two prep units and the sushi cutting board are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness*
    Observation: Chlorine sanitizing solution used was not at an acceptable concentration. Wiping buckets concentration was observed at 10ppm plus detergent is being added to solution. Do not add detergent.
    Correction: Adjust the chlorine sanitizing solution to a level of at least 25 parts per million when the pH is at 10 or less and the water temperature is at 120°F. At room temperature concentration should be 50 to 200ppm

Several notes were made and discussed with management during the inspection and are as follows
  • Critical: Demonstration of Knowledge*
    Observation: Several employees have expired Western Tidewater Food handler cards and two kitchen staff do not have cards
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.
  • Equipment - Good Repair and Proper Adjustment
    Observation: sushi display was observed in a state of disrepair and damaged, rear sliding doors cracked in broken allowing moisture in.
    Correction: Repair the sushi display to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the sushi display, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

- Improvements must be made on date labeling all opened products and ensuring that proper cooling methods are being practiced.
- Keep all food products covered when not in use.
- Vent hood needs to be cleaned.
- Ice machine needs deep cleaning.
- Remove fly strip and clean ceiling fan.
- Install self-closing and tight fitted door at exit in kitchen to prevent a pest problem.
- Store wet wiping clothes in sanitizing bucket between uses.
- All staff need current health cards.

  • Critical: Hands - Preventing Contamination from Hands*
    Observation: Observed scoops with no handles found in dry food prodcuts.
    Correction: Replace with scoops that have handles.
  • Food - Packaged and Unpackaged - Separation, Packaging, and Segregation
    Observation: Observed salmon wrapped in dry paper towels near sushi display.
    Correction: Keep food wrapped in approved food-grade material until prepared, cooked or served.
  • Utensils - In-Use - Between-Use Storage (corrected on site)
    Observation: In-use utensils improperly stored between use.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.
  • Food Storage - Clean and Dry Location
    Observation: Food stored on the floor or food stored less than 6" above the floor.;Elevate food storage onto approved shelving with minimum 6" legs or casters.
  • Critical: Cooling* (corrected on site)
    Observation: Rice from reach-in refrigerator noted not being adequately cooled to prevent the growth of harmful bacteria. PIC immediately removed and discarded product.
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses.
  • Critical: Food - Potentially Hazardous Food - Hot Holding* (corrected on site)
    Observation: Rice hot holding at improper temperatures.
    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking*
    Observation: The prepared ready-to-eat (RTE) in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: can opener in kitchen.
    Correction: Clean and sanitize these surfaces for food contact.

- All staff are must have valid health cards from Western Tidewater Health Department as required by Isle of Wight County Code.
- Records of parasite destruction available and presented to EHS. Great job!
