    Observation: The person in charge failed to state the minimum cook temperatures and times for potentially hazardous food. Asked PIC cook temperatures for fish and he did not know correct temperatures. Left PIC handout of cook temperatures.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.

    Observation: The person in charge failed to describe the symptoms associated with diseases that are transmissible through food. EHS asked PIC about symptoms related to illness and he could not state the symptoms. EHS left handout with symptoms and reportable disease.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements.     Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food, including the date of onset of jaundice or of an illness due to Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus. EHS asked PIC what the five reportable diseases are, and he could not answer. Left PIC handout.
    Correction: Have the license holder require employees or applicants report to the person in charge any information about their health and activities if they are diagnosed with or had a past illness from Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site) (repeated violation)
    Observation: Unwrapped or uncovered food in the prep refrigerator. EHS observed uncovered Fries, fish, shrimp and chicken inside the prep refrigerator. PIC covered the foods.
    Correction: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
  • Food - Food Labels (corrected on site)
    Observation: Consumer self-dispensing bulk food bin containing flour and spices is not properly labeled. EHS observed several containers of flour and spices that were not labeled.
    Correction: Bulk food that is available for consumer self-dispensing shall be prominently labeled with the following information in plain view of the consumer: 1. the manufacturer's or processor's label that was provided with the food or, 2. a card, sign, or other method of notification that includes the information specified in this section.
  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located in the prep refrigerator. EHS observed that the prep refrigerator did not have a thermometer.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket to the reach in refrigerator is in poor repair. EHS observed a torn gasket on upright True refigerator.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
  • Non-Food Contact Surfaces (repeated violation)
    Observation: The nonfood contact surface of the Upright True freezer and gasket to glass door refrigerator has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: The sink basin at the three compartment sink is slow to drain. EHS observed three compartment sink slow to drain.
    Correction: Plumbing systems and components shall be maintained in good repair.

    Observation: Three compartment sink faucet in poor repair. EHS observed three compartment sink faucet leaking.
    Correction: Repair and maintain all plumbing components ans fixtures.
  • Ventilation Systems - Cleaning Ventilation Systems - Nuisance and Discharge Prohibition (repeated violation)
    Observation: Vent filters in the hood system are not being maintained in a clean condition. EHS observed dust build up on hood system filters.
    Correction: Maintain hood system vent filters in a clean condition.
  • Ventilation Systems - Cleaning Ventilation Systems - Nuisance and Discharge Prohibition (repeated violation)
    Observation: Intake and exhaust air ducts are not being cleaned. EHS observed ceiling vents for HVAC systen have dust build up.
    Correction: Clean intake and exhaust air ducts and change filters so they are not a source of contamination by dust, dirt and other materials
  • Mops - Drying Mops (repeated violation)
    Observation: Mops not hung up to air dry. EHS observed mop drying in mop bucket. EHS educated to hang mops to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

One food handler card has been obtained and the other is obtaining card today per PIC. Good work on correcting almost all discrepancies. Maintain facility clean for the future in preventing food borne illnesses. .

06/18/2015Follow-up
Many discrepancies are noted in previous inspection. Facility has made no effort to correct them. Permit expires and of June 2015. PIC has been notified that all discrepancies must be corrected before new permit will be issued. Food Hnadler cards must be obtained right away.

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: The handwash station at the kitchen is being used for purposes other than washing hands. EHS observed trays drying in the handwashing sink.
    Correction: The handwash facility identified above is to be used for washing hands only

    Observation: No disposable towels were provided at the hand washing lavatory in the kitchen
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Correction: All personal care items, medicines and first aid supplies must be stored in such a way to prevent potential contamination of food, food contact items, equipment, utensils, linens, single-service and single-use articles.

Obtain food handler cards as required. Overall general cleaning needs to be accomplished. Recommend establishing a cleaning schedule to address and maintain proper cleaning. Obtain correct test strips for the sanitizer. EHS discussed proper date marking for prepared salads and sauces and other food items.

When asked food workers (owners) stated they had no employee health policy in place. FDA 1B form was provided for future use. EHS also discussed the form and what illnesses food workers must be restricted from working with food.

    Observation: The prepared ready-to-eat (RTE) sauces, prepared salads and crab egg role in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Warewashing Equipment, Determining Chemical Sanitizer Concentration
    Observation: A sanitizer test kit is not being used to ensure the chemical sanitizing solution(s) are at the proper concentration. Observed incorrect test kit for the type of sanitizer being used in facility.
    Correction: Use a QT chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the microwave, deep fryers, hood vent counter on storage table has accumulations of grime and debris creating harboring conditions for pest and rodents.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Wiping cloths improperly stored between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
