The purpose of today's visit was to conduct a routine assessment. The following suggestions have been made as a means to incorporate Active Managerial Control: 1. Have multiple Certified Food Managers (CFMs) assigned to a shift in the event that one needs to leave food establishment 2. Train employees to take receiving temperatures for all items being delivered to the store (41F or below for all potentially hazardous foods being cold held) A follow-up inspection will be conducted by 9/18/15 to ensure a CFM is on-site during hours of food preparation and service. Establishment is temporarily closed.

Observation: There is no Certified Food Manager (CFM) present in the establishment.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM NOT PRESENT DUE TO A PERSONAL EMERGENCY. THERE IS NO OTHER CFM AVAILABLE. ESTABLISHMENT IS TEMPORARILY CLOSED.
- Refuse Container with Cover/ Women's & Unisex Toilet Room
Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
Correction: PLEASE PROVIDE COVERED TRASH RECEPTACLE IN LADIES RESTROOM WITHIN 30 DAYS. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at handwashing sink NEAR FRONT SERVICE AREA used by food employees.
Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. HAND WASHING SIGN PROVIDED.

This is a complaint investigation. Observed waste water overflowing from service lateral line onto the parking lot and flowing near the storm drain. Waste water discharge is public health menace and hazard. This facility is voluntarily closed. **Please contact Health Department when piping is fixed and solid waste is disposed.

Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
