    Observation: Reach-in cooler number 1 was not maintaining proper cold holding temperatures (No PHFs).
    Correction: Repair as necessary to maintain food items at 41*F. Operator removed all potential hazardous foods and contacted the repair department to service the unit.

    Observation: Goose neck on the faucet at the preparation sink observed with a leak.
    Correction: Repair.
  • Critical: Backflow Prevention, Air Gap*
    Observation: An air gap between the water supply outlet and the flood rim level of the ice maker is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

    Observation: Two bags of mozzerella cheese was found with mold..
    Correction: Ensure food is safe and unadulterated. Operator discarded the cheese.

    Observation: There was no temperature measuring device located in the following equipment:
    >> Food warmer
    >> Milk cooler

    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

    Observation: Single service items located on the serving line observed stored face up with food contact surfaces exposed.
    Correction: Store single service items in its original protective packaging or inverted in an approved dispenser.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections on the prep sink are leaking. When the water is turned on, the top of the goose neck sprays water
    Correction: Plumbing systems and components shall be maintained in good repair.

All temperatures were taken internally. No PHFs were being served on today's Menu. Hot dogs Hamburgers, carrot sticks, and salad. School was participating in fun day activities.

  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: Serving line tray slider which is made of wood was observed with protective coating deteriorating.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent. Repair or replace the tray slider. Person in charged did mentioned that a new one has been ordered and she should receive it some time this summer.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket to the upright food warmer was dry rotted and torn on the left door.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
  • Equipment - Cutting Surfaces (corrected on site)
    Observation: Two cutting boards found stored on the bottom shelf of a preparation are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized. Person in charged discarded the cutting boards and request new ones via phone call during the time of the evaluation.
  • Non-Food Contact Surfaces
    Observation: The following nonfood contact surface of the has accumulations of grime and debris:
    1. Hobart Freezer gaskets was moldy. (corrected)
    2. McCall Freezer gaskets had food debris.(corrected)
    3. Ventilation fan in mop room area had accumulations of dust.
    4. Wooden tray slider on the serving line had food debris in between the center crevice.

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities in Good Repair
    Observation: The following physical structure are not maintained in good repair:
    1. Dry storage room observed with cracks and not sealed to the ceiling.
    2. Inside and outside of walk-in cooler door frame noted with various cracks.
    3. Floors in dishwasher room, under steamer, and 3 compartment showing signs of deterioration.( with rust).
    4. Ceiling in the employee rest-room was not sealed to adjourning walls.

    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Repair all surfaces that may need sealed or resurfacing.

    Correction: Install sneeze shield.

    Observation: The unused mixer is leaking lubricating fluid.
    Correction: Repair.

    Observation: The following items had discrepancies:
    >> Paint peeling on the hood of the automatic warewashing machine.
    >> The wooden surface on the main serving line is deeply scored and chipped.
    >> Paint peeling from the hood system of the ovens.
    >> The gaskets on the Cres Core unit is cracked.
    >> The plastic vent cover on the two door freezer is cracked.
    >> There was a hole in the drip pan of the oven.
    >> The mop room sink basin has rust.

    Correction: Repair or replace.

    Observation: There were 2 cracked plastic pitchers.
    Correction: Replace.

asked manager if wraps or salad cups ( with cut tomato ) were to be returned to refrigeration unit after lunch -- was told NO . They were to be consummed or discarded after lunch period . The policy and procedures manual needs to state that unless temperatures taken of the product and < 41 ( and documented ) then salads / sandwiches - wraps and salads or fruit cups featuring cut tomatoes or cut cantalope must be discarded after lunch period .
Due to the age of the school , the facility does not comply with current code requirements .In the event of a change of operator or remodeling , the facility must be brought up to the current Code requirements . This will include the addition of one of more handsinks , improved lighting , repair of structural deficiencies , repair or replacement of damaged equipment and proper installation of laundry facilities ,
Told dish person they must record both wash and rinse temperatures . Advised that the rinse was the more important of the two ( had been only recording the wash temperature )

  • Food Display
    Observation: open tray of rolls out on the serivng line not under the sneeze guard
    Correction: advised to place the rolls under the sneeze guard . Told manager that portion cups with lids could sit out on the serving line not being under the sneeze guard if space was needed under the guard ( which it was not )
  • Bearings and Gear Boxes, Leakproof
    Observation: large mixer ( bread corner ) is leaking lubricant
    Correction: do not use mixer until repaired and not leaking lubricant . IF IT IS NOT USED AT ALL REMOVE IT FROM FACILITY
  • Equipment - Good Repair and Proper Adjustment
    Observation: gaskets of the CRESSCOR hot holding cabinetry unit doors are split
    Correction: monitor condition . If the damage worsens gaskets are to be replaced
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: mixer bowl and splatter areas of the mixer in the bread corner are dirty from leaking lubricant
    Correction: if the mixer is repaired the mixer bowl and splatter areas of the mixer are to be cleaned and sanitized prior to use
  • Non-Food Contact Surfaces
    Observation: tops of hot hold cabinetry and oven are dirty / dusty
    Correction: clean
  • Physical Facilities in Good Repair (repeated violation)
    Observation: holes in ceiling ( 2 pairs ) along window wall where piping installed almost a year ago
    Correction: restore ceiling to smooth - durable - non absorbant and easily cleanable

Repair for dish machine called during inspection

  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The following equipment is in poor repair:
    -heater vent in store room is rusted
    -side of oven has paint peeling/some corrosion
    -out side of mop sink has paint peeling
    -seal on door of walk in cooler is damaged
    -dish machine not reaching spec. of 180* during rinse (although t-stick confirmed that internal temp of dish is reaching 160*) temp . chart confirmed rinse cycle reaching only 173-179 in the last 6 weeks.

    Correction: Repair or Replace
  • Non-Food Contact Surfaces
    Observation: The fan attached to wall to the right of RC1 needs cleaning
    Correction: Clean
  • Plumbing System Maintained in Good Repair
    Observation: The cold water faucet at the handsink in kitchen has a leak.
    Correction: Repair and maintain all plumbing components and fixtures.
  • Outer Openings - Protected (repeated violation)
    Observation: Screen door is torn.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: The following structures are in need of repair:
    -floor under mixer stained
    -floor in dish machine room has scale build-up
    -open holes in ceiling in kitchen
    -wall in locker room cracked
    -wall to right of vent hood in dish room cracked
