    Observation: Observed faucet on handsink was loose and wobbly.
    Correction: Repair and maintain all plumbing components ans fixtures.

- Rehabilitation side inspection performed.
- All staff wearing hair restraint properly.
- No food service or food preparation observed during inspection.
- All food is wheeled over from larger kitchen, same for wiping buckets.
- No leftovers and no cooling for reheating.
- Discussed that chemical sanitization strips also be stored on rehabilitation side of facility.
- Kitchen observed clean with no signs of deep cleaning needed.
- If any upgrades/renovations are performed in this kitchen, notify the Suffolk Health Department prior to this change.
- All staff are up-to-date with food handler cards.

- Food temperatures taken often with stem thermometers.
- Wiping buckets tested at proper chemical sanitization level.

- Proper glove use observed with cutting ready-to-eat foods on cutting board.
- Discussed Employee Health policy. All staff are up-to-date with district food handler cards.

    Correction: Informed PIC that all staff must be up-to-date with FH cards ASAP. PIC agreed to have all staff up-to-date in 30 calendar days.

    Observation: The cutting board(s) located in the main kitchen are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
