Food suppliers are US Foods and Kroger. Discussed employee health policy, Form 1B is used. Recommend cleaning the cavity of the microwave and the shelving under the prep table. Observed overall clean facility.
  • Hair Restraints - Effectiveness (repeated violation)
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food
  • Equipment - Non-Food Contact Surfaces and Utensils
    Observation: Surfaces of the interior of the ice machine was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of interior of the ice machine at any time when contamination may have occurred or per manufacturer specifications.
  • Plumbing System Maintained in Good Repair
    Observation: The 3 compartment sink observed in poor repair.
    Correction: Repair and maintain all plumbing components ans fixtures.

Food suppliers are US Foods & Kroger. All foods precooked, besides raw ground beef for sloppy joes & spaghetti sauce. No cooling of foods, leftovers are discarded or given to staff. Discussed cleaning/sanitizing food contact surfaces. Discussed Employee health, Form 1B signed & posted in facility. Overall clean facility.

  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Observed sliced ham and butter cold holding at improper temperatures
    Correction: observed sliced ham at 48 F and butter at 46 F. PIC was not aware why or how long these food items had been cold holding > 41 F and no temperature logs were kept. All other food checked in the unit was observed properly cold holding.

Form 1B posted and in use. Fire suppression system serviced November 2013. Western Pest is the LPCO. Pasteurized fruit juices used. Observed good glove use and hand washing. Discussed date marking.

No food prep at time of inspection (corn dog nuggets just put into oven). US Foods and Kroger are suppliers. Leftovers are either given to staff or discarded. Most proteins received pre-cooked and frozen, only the ground beef for hamburgers/ meatloaf is not received pre-cooked. Food is portioned out in the lunch room by staff
  • Person in Charge
    Observation: Observed no means to verify that employees are trained on the employee health policy and their responsibility to report to the person in charge (PIC) certain symptoms, diagnosed illnesses and exposures associated with foodborne illness.
    Correction: Maintain documentation to verify that employees have been trained on the employee health policy and their responsibilities regarding the policy. EHS provided Form1B.
