A complaint inspection was received stating dirt and grime on walls, floor and trash containers. During the inspection the facility was observed clean. Person in charge was unaware of the complaint. Operator reminded to keep the facility clean and in good sanitary condition.

  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Breakfast burrito (egg and sausage) cold holding at improper temperatures in that said food registered at 45 degrees F in reach in refrigerator next to 3 compartment sink. Observed the door of said unit was not closed completely.
    Correction: Operator voluntarily relocated food to the walk in refrigeration unit that was holding food at 39 degrees F.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The inner panel of ice machine observed broken.
    Correction: Replace the inner panel to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
  • Handwashing Signage/Handwashing Facilities (corrected on site)
    Observation: A sign or poster that notifies food employees to wash their hands is not provided in employee restrooms.
    Correction: Operator voluntarily provided a temporary sign that notifies food employees to wash their hands at said handwashing lavatories.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Accumulation on standing water (pool of water) observed next to laundry machine and under soda dispenser in basement food/dry goods storage area.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

A complaint was received stating that customers observed ants crawling all over the floor in lower dining area and in ladies restroom.
Manager was unaware of the complaint. During the inspection, ants were observed in the ladies restroom and in customer dining area.
Manager on duty was aware of the issue and had pest control service done on 6/23/15. Pest control report was provided. Continue working with pest control services. Cracks between the tiles and wall junctions were observed in the restrooms and in dining area. All the cracks must be sealed.
Eliminate the pest and its harborage condition in the facility.
Complaint addressed.
Note: As per the manager the dining area, lobby and restrooms will be remodeled after 2 weeks.

1. Water leaking from ceiling in the basement above storage racks. Must be repaired.
2. Ice scoop holder must be provided.
3. Handle for walk in freezer observed broken. Must be replaced.
4. Broken, cracked and missing tiles observed on floor, floor wall junctions next to the stairs/door to basement storage room, on floor wall junction on the side of walk in freezer. Tiles missing under 3 compartment sink, under/next to handwashing sinks in main floor. Seal the gaps between tiles and floor/wall in main floor and basement. Cracked and broken tiles under ice machine and next to back door. The floor under the soda machine must be leveled to avoid standing water.
5. Replace the unit thermometer for 4 door reach in refrigerator in main floor.

    Observation: Observed raw shell eggs not stored in refrigeration equipment that maintains ambient air temperature of 45°F or less in that said food observed stored in walk in refrigerator with ambient temperature of 55 degrees F.
    Liquid eggs, milk, chocolate milk, creamer, sliced tomatoes registered at 55-58 degrees F in walk in refrigerator. Said unit registered operating at 55 degrees F. As per the manager said unit has been functioning at said temperature from last night (12 hours prior to inspection)

    Correction: Store raw shell eggs in refrigerated equipment capable of maintaining ambient air temperatures of 45°F or less.
    Operator instructed to discard all potentially hazardous food held in said unit overnight. Repair the unit to hold potentially hazardous food at adequate temperature of 41 degrees F or below.
  • Critical: Time as a Public Health Control* (corrected on site)
    Observation: Observed sliced tomatoes intended to be stored for 6 hours at ambient temperature did not have an internal temperature of 41°F or below. Improper labeling in that label stated that said food would be held for more than 6 hours.
    Correction: Assure sliced tomatoes to be stored at for up to 6 hours has an internal temperature of 41°F or below and properly labeled

    Observation: Observed accumulations of mildew on the inner panel of ice machine.
    Correction: Clean and sanitize these surfaces for food contact.

    Observation: The handwash facility observed broken and not in use therefore, no handwash lavatory at kitchen, preventing routine handwashing by food workers.
    Correction: Repair and maintain the handwashing facility in kitchen so that the employee have readily accessible handwashing facility at all times.

A complain was received stating that when the customer complained about soggy fries, the employee took the bag of fries (which was contaminated as it was dumped by the customer earlier) and put them back in the fries dispenser on left side. Manager on duty was not aware of the complain. During the inspection none of the employees took back any served food or reserved any served food. As per the manager the oil in all three fryers are changed everyday. Manager will retrain their staff on not accepting or reserving of previously served food as it may be contaminated.
Complain was addressed.

    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands. Operator voluntarily provided soap in said hand washing facility.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatories in the kitchen/active food prep areas.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials. Operator voluntarily provided paper towels in both handwashing sinks.
