Maintain the corrected items as corrected. Observed missing cove base and tiles at/neat mop station -please repair as needed within 90 days. Gasket on cooler in poor condition - replace.
Critical: Employee Health* (corrected on site) Observation: When asked, the Person in Charge was not able to describe or locate an employee health policy.
Correction: Train all staff on the provided employee health policy (EHS reviewed policy with PIC).
Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition* (corrected on site) Observation: According to the date mark the cooked mushroom should have been previously discarded.
Correction: Promptly discard foods that have reached the end of the seven day holding period.
Critical: Equipment and Utensils - Before Use After Cleaning* (corrected on site) Observation: The sanitizer in the three vat sink was measured at less than 150 ppm quat.
Correction: Provide sanitizer at the concentration directed by the manufacturer for sanitizing (keep test strips in an accessible location).

Correction: Instruct all food service workers to remove soiled gloves prior to hand washing.
Critical: Temperature, Cooking - Raw Animal Foods* (corrected on site) Observation: Improperly cooked food - burger ordered well done.
Correction: Continue cooking the food until it has reached 155°F or above for 15 seconds.
Critical: Cooling* Observation: Improper cooling - one step.
Correction: Cool foods prepared from ingredients at ambient temperature to 41°F within 4 hours.
