  • Utensils - In-Use - Between-Use Storage
    Observation: A metal ice scoop was observed with the handle beneath the ice.
    Correction: Ice scoops may be stored in the ice with the handle above the ice or on a clean, sanitary surface.
  • Warewashing Machines, Manufacturers' Operating Instructions
    Observation: A manufacturer's specifications plate was not found on the SSDC warewash machine.
    Correction: A warewashing machine must be operated in accordance with the machine's data plate and other manufacturer's specifications.
  • Warewashing Equipment, Clean Solutions
    Observation: The in-use (wash, rinse, sanitize) water in the warewashing sink was observed to be heavily soiled or contaminated.
    Correction: Drain the basin(s) and refill with fresh water and cleaning agents/sanitizers added as required for effective cleansing, rinsing and sanitization of equipment and utensils.
  • Warewashing - Mechanical Warewashing Equipment, Wash Solution Temperature
    Observation: The temperature gauge on the warewash machine is not giving an indication of the machine's water temperature.
    Correction: The temperature of the wash solution in spray type warewashers that use chemicals to sanitize shall not be less than 120 degrees.
  • Refuse Outside Receptacles
    Observation: The outside refuse area was observed in a very unsanitary condition, with bags of trash and other loose trash littering the area. A large area of asphalt around the two waste oil containers was observed blackened by a build-up of oil and grime.
    Correction: Receptacles and waste handling units for refuse must be maintained so that accumulation of debris and harborage and attraction of insects and rodents is minimized, and effective cleaning is facilitated.
  • Refuse - Covering Receptacles
    Observation: The waste oil and grease container in the outside refuse area was observed with the lid in the open position.
    Correction: Outside refuse lids must have tight fitting lids and must remain closed to prevent attraction and harboring of insects and other pests.

- Onions on the floor in the shed/dry storage area need to be stored properly off the floor.
- The dish machine needs to be repaired before using again. Use the 3 basin sink until the dish machine is repaired properly.
- The hand sink should be re-installed in the next 10 business days. A re-inspection will be needed to verify the sink and dish machine repairs.
- The original hand sink spout was addressed last inspection and should be repaired by the re-inspection date also.
- Find a way to address the raw steak for steak and cheese in the prep unit so it can not cross contaminate vegetables or cheese on the prep unit.

  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit. (Chicken in the walk-in)
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level for the dish machine.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness*
    Observation: Chlorine sanitizing solution used was not at an acceptable concentration.
    Correction: Adjust the chlorine sanitizing solution to a level of at least 50 parts per million when the pH is at 10 or less and the water temperature is 100°F.
  • Critical: Plumbing - Approved System and Cleanable Fixtures*
    Observation: The (faucet/fixture) on the hand sink was not designed for the smaller sink and should be replaced.
    Correction: Repair or replace the faucet spout to provide proper operation.
  • Critical: Handwashing Lavatory*
    Observation: The hand washing sink was removed from the cook/prep line preventing routine handwashing by food workers.
    Correction: Re-install a handwash lavatory on the prep line where previously plumbed to allow convenient use by food workers.

- Install paper towel dispenser and new spout on hand sink.
- Provide ammonia based sanitizer test strips and chlorine test strips. (Chlorine test strips present, but the whole roll got wet.)

-Provide the correct faucet spout on the hand sink across from the dish machine.
- The dish machine has a buzzing noise when dispensing chemicals, have the machine checked to ensure it is working correctly.
- Provide chlorine test strips for the dish machine and quaternary ammonia test strips for the 3 vat sink.
- More shelves are ordered for the walk-ins. Food product must be kept off the floor.
- Temporary towel dispenser on the cook line until the automatic dispenser comes in.

No violation noted during this evaluation.
09/26/2013Pre-Opening
- Hand sink faucet will be replaced with a spout that better fit the sink.
- Stock all hand sinks with soap and towels.
- Provide signs for bathroom sinks that say Employees must wash hands.
- Provide a covered trash can in the lady's rest-room.
- Provide a mop drying rack.
- Make sure the dish machine operates correctly and sanitizer levels are correct.
- Silicone caulk joint at the wall where the 3 vat sink meets.

No violation noted during this evaluation.
