The purpose of today's visit was to conduct a risk factor assessment. Please continue to incorporate the following practices as a means to demonstrate Active Managerial Control:
1. Regular food safety training for all employees
2. Employee hygiene training
3. Monitoring handwashing procedures
4. First-In-First-Out (FIFO) rotation of refrigerated and dry storage items
5. Maintaining and monitoring of food temperature logs

The purpose of today's visit was to conduct a risk-factor assessment. Discussed active managerial control (AMC) and shared responsibility. Currently displaying AMC through the following practices: date marking of all items received during shipment to facilitate First-In-First-Out rotation, equipment temperature logs for each refrigeration unit, sanitizer concentration logs, and refrigeration of all canned goods to quickly cool potentially hazardous foods. Also suggested the use of food temperature logs to maintain at cook-line to monitor minimum cook and re-heat temperatures. Please continue to use AMC in our establishment to promote public health.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required. OBSERVED PINK MOLD ACCUMULATION ON INTERIOR OF ICE BIN
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. PLEASE WASH, RINSE, AND SANITIZE INTERIOR OF ICE BIN REGULARLY TO AVOID MOLD ACCUMULATION.

Please be advised to monitor the ambient air temperature of Air Screen cold holding unit to ensure that it is maintaining a temperature of 41F or below.     Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: AIR SCREEN COLD HOLDING UNIT -- MILK (50F), MILK (50F).
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. ITEMS IN AIR SCREEN COLD HOLDING UNIT WERE MOVED TO A DIFFERENT COOLER. AIR SCREEN TEMPERATURE WAS RESET TO BE 41F OR BELOW PER DISCUSSION WITH PERSON IN CHARGE.

    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

    Observation: The following cooked potentially hazardous food(s) are not being adequately cooled from 135°F to 70°F within two hours and to 41°F within a total of six hours.--OBSERVED COOKED TURKEY LEGS IN THE WALK IN COOLER AT 52F. CFM STATED TURKEY LEGS HAD BEEN COOLING OVERNIGHT.
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within two hours and to 41°F within a total of six hours to prevent the growth of harmful bacteria. Foods that have been temperature abused are to be discarded.--CFM DISCARDED TURKEY LEGS.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: VICTORY 2DR
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. PLEASE FAX COPY OF REPAIR INVOICE FOR THE VICTORY 2DR COOLER. DO NOT PLACE PHF IN THE UNIT UNTIL IT CAN MAINTAIN 41F OR BELOW.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site) (repeated violation)
    Observation: The following equipment/utensils were observed soiled to sight and touch: OBSERVED SOILED VEGETABLE PEELER IN CLEAN UTENSIL DRAWER AND SOILED ICE CREAM SCOOP IN CLEAN UTENSIL CONTAINER.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.--CFM PLACED SOILED UTENSILS IN THE THREE COMPARTMENT SINK.
