    Observation: Raw and/or undercooked meat and fish are provided on the menu, menu board, table tent or brochure without proper disclosure.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods and identification of the animal-derived foods by asterisking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Sanitizer dispenser was observed in a state of disrepair. Sanitizer was not coming through the pipe.
    Correction: Repair dispenser to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Equipment - Cutting Surfaces (repeated violation)
    Observation: The cutting board along the dessert make-up table is heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Food stored in a location where it is subject to splash, dust or other contamination. Bananas stored on the hook right above 3 VAT sink.
    Correction: Store food where it is not exposed to splash or install an approved durable, and cleanable barrier between the splash source and the food to prevent contamination.

    Observation: Half and half creamers that required refrigeration stored (complimentary table) at improper temperatures
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: According to the ""consume by"" date on the commercially processed ready-to-eat (RTE) roast beef and other sandwich meats in the walk-in cooler, the food should have been discarded 20 days ago.
    Correction: Discard the food at this time and ensure all commercially processed RTE food is served, sold or discarded by the ""consume by"" date after opening. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.

Ready-to-eat foods must be discarded within seven days.
Cooling methods should be addressed with staff immediately to ensure proper cooling.

    Observation: Mashed potatoes and macaroni & cheese made the previous evening noted not being adequately cooled to prevent the growth of harmful bacteria. Foods were voluntarily discarded.
