    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

Delfield reach-in had been turned up to keep foods from freezing.
Hand washing and glove changing procedures has dramatically improved.
Continue to train to employees on hand washing and changing gloves. Recommended self inspections daily.

Facility has adequate metal stem thermometers and quaternary ammonium test kit.
Employee health policy MUST be implemented immediately! Once again provided operator with FDA form 1-B. ALL employees must be familiar with the five major foodborne illnesses/symptoms and reporting procedures.
Do not use the delfield reach-in cooler on the line until it has been repaired.
Review glove changing procedures will ALL employees.
Recommended doing self inspection daily to ensure compliance.
A follow-up inspection will occur on/about December 20, 2012.

    Observation: The same gloves were used to sanitizer counter, handle raw sausage patty, and bagel.
    Correction: Use only cleaned and sanitized utensils or equipment during food preparation.

    Observation: Cheese and Canadian bacon cold holding at improper temperatures. Foods were moved to the walk-in cooler.
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.

    Observation: Several foods in the warming drawers for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time. Review procedures for hold times on cheese.
    Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time.
  • Temperature Measuring Devices
    Observation: Thermometers are needed in the reach-in freezer & reach-in cooler on the cook line, the delfield salad cooler, Bev air freezer, and front reach-in salad cooler. Thermometer was placed in the cooler on the cook line.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Reach-in Delfield cooler on the cook line observed with an ambient air temperature of 50 F. Several reach-in freezers observed with heavy accumulation of ice and the door of the reach-in freezer by the french fry fryers does not seal properly.
    Correction: Repair the refrigeration to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the refrigeration, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Equipment - Non-Food Contact Surfaces and Utensils
    Observation: Surfaces of the soda nozzles was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of soda nozzles at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Handwashing Lavatory, Water Temperature, and Flow (repeated violation)
    Observation: Water from the handwashing sink at by the ice machine was measured at a temperature less than 100°F. (75 F)
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: Heavy accumulation of food/grease observed on the floor under the grills.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Burrito mixture cold holding at improper temperatures. Food was voluntarily discarded.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: Food items in drawers and food items on prep line for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.
    Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time.

    Observation: Soap was not provided at the hand washing lavatory in the prep area.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
