    Observation: Observed no consumer advisory linked to the burgers which can be served undercooked.
    Correction: Provide a disclosure statement on the menu with a mark indicating the foods that are served not fully cooked.
  • Critical: Backflow Prevention Device, When Required* (repeated violation)
    Observation: Observed the "Y" fixtures attached to the mop sink do not have a backflow device in front of them.
    Correction: Provide a double check valve backflow device before each "Y" fixture on the mop sink.

    Observation: Observed the lights in the walk-in refrigerator did not have a protective shield.
    Correction: Provide a protective shield for the lights.

    Observation: Observed the gasket torn on the right prep unit on the line.
    Correction: Have gasket replaced (already ordered).

    Observation: Observed the "Y" fixtures attached to the mop sink do not have a backflow device in front of them.
    Correction: Provide a double check valve backflow device before each "Y" fixture on the mop sink.

    Observation: Observed the baseboards were starting to come off the walls in several parts of the kitchen. Also observed a crack in the main bar (side closer to main doors) that needs to be sealed.
    Correction: Replace/Repair the baseboards throughout the kitchen where they are coming off.

    Observation: Observed the lights in the walk-in refrigerator did not have a protective shield.
    Correction: Provide a protective shield for the lights.

    Observation: Observed the floors in the bars, the walk-in refrigeration units, and in the kitchen are in need of cleaning.
    Correction: Clean the floors in the kitchen and the bars, especially under the tables and equipment.

    Correction: Have pest control company come and treat for drain flies.

    Observation: Coving along the make line and throughout is coming loose.
    Correction: Replace or repair the coving throughout the kitchen to prevent food and other debris from collecting behind them.

    Observation: Right prep unit on the line was warm. Food was between 55- 60 degrees.
    Correction: Discard food that was kept in there over night, any food moved there this morning was returned to the walk-in cooler.
  • Food - Food Labels (corrected on site)
    Observation: Bulk containers of ingredients not labeled.
    Correction: Label all bulk containers so that the ingredients are easily identifiable.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Gaskets on the left prep unit and the fry freezer are torn.
    Correction: Replace gaskets/seals on refrigerations units that are torn.
  • Equipment - Non-Food Contact Surfaces and Utensils (corrected on site)
    Observation: Soda dispensers at front bar were soiled on the inside.
    Correction: Clean the surface of soda dispensers at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications. When soaking them overnight, make sure to scrub the inside to prevent build-up of mildew.

Sanitizer levels were good and in use.Gasket on the door of the freezer by the fryer is ripped, gasket needs to be replaced. Make sure pans and dishes are completely dried before stacking them.

    Observation: Slicer was soiled, can opener was soiled, and clean dishes were found soiled.
    Correction: Make sure dishes are cleaned completely before placing on clean racks. Make sure the slicer is broken down and cleaned thoroughly after use. Make sure to clean the can opener regularly.
  • Equipment - Non-Food Contact Surfaces and Utensils (corrected on site)
    Observation: Back bar soda dispensers and holders were dirty. Ice scoop at server station had mildew on it.
    Correction: Clean and sanitize the holder and dispensers in the back bar. Make sure the ice scoop is cleaned and sanitized daily.
