- Food Contact Surfaces - Cleanability*
Observation: The food contact surface of the counter top is not designed or constructed to be smooth and it contains cracks, chips, or pits that can not be easily cleaned. (the caulking between ledges/ back splah areas is not smooth and easy to clean). (All the caulking is in the process of being redone).
Correction: Replace thecaulking to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.
- Equipment - Good Repair and Proper Adjustment
Observation: Unused or non-functioning equipment not removed from the premises. Tools from the tool box were all sitting out on the counter
Correction: Remove any unused or non-functioning equipment from the premises.

Observation: The nonfood contact surface of the @EQUIPMENT@ has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: Clean carry out trays (inverted) were observed stored uncovered and unprotected from a working prep area above them.
Correction: Store equipment and utensils covered or inverted to prevent contamination while in storage.

Correction: Keep handwashing facilities clean and maintained to encourage proper handwashing.

Observation: Refrierator door gasket was observed in a state of disrepair and damaged.
Correction: Repair the refrigerator door gasket to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair thegasket, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

(tiles are popping up).

- Water Distribution, Delivery, and Retention System
Observation: The (nonpublic water main, water pumps, pipes, hoses, connections, water transport vehicle, water container) is not (constructed, maintained, operated) according to requirements of the Health Department. (pump is leaking water).
Correction: New pump is on order (receipt available). Water is being boiled on stove top. Water is from an approved source.EHS is to confer with his/her supervisor and follow-up on the conditions at the food service facility to determine closure of the establishment or cessation of certain food service operations due to an imminent health hazard. (See section 12 VAC 5-421-3910)
- Physical Facilities - Cleaning Frequency and Restrictions
Observation: Floor areas beteween eqipment need cleaning and are noted in need of cleaning.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Observation: Supply hooks for mops, broom, dust pans, etc.
Correction: Maintenance equipment supplies and tools such as brooms, mops, vacuum cleaners, and similar items must be stored so they do not contaminate food equipment, utensils, linens, and single-service and single-use articles.
