Observation: Tags missing from the bags of mussels. The tags for the clams did not had a date for the time when the last clam was sold.
Correction: Ensure the tags remain attached to the shellstock container in which they were received until the container is empty.
- Sanitizing Solutions, Testing Devices
Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a @SANITIZING AGENT@ test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

Observation: Dishwashing machine not observed sanitized:.
Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.

Observation: Backflow or backsiphonage prevention device not available on threaded faucet in the mop sink.
Correction: Provide a backflow or backsiphonage prevention device on all threaded faucets.
- Handwashing - Using a Handwashing Lavatory
Observation: The handwashing facility located at the right side of the three compartment sink is blocked (grocery car), preventing access by employees for easy handwashing.
Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the grocery card preventing its use.

Observation: Outer opening of the food establishment is not protected against entry of insects and rodents.Screen door is not a tight fit.
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Physical Facilities in Good Repair
Observation: Tile missing in the floor in front of the walk in cooler.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Pests - Controlling Pests*
Observation: Two baby cockroaches observed in the wall of the dishwashing area.
Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
