Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.

Observation: The floor drain at the three vat vink was dirty.
Correction: Clean the floor drain.

Correction: Seal floor and wall juncture in rooms where water flush cleaning methods are used. Replace broken and missing floor base .

Observation: Dead or trapped flying insects were found in fly traps on the premises The glue board had not been removed since May 2013.
Correction: Remove dead insects, and other pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
