Proper hand washing observed and date marking observed.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Pipe connected to pressure gauge for rinse cycle observed leaking water and showing lower than normal water pressure, thus observed in a state of disrepair and damaged. Call made to maintenance for repairs.
    Correction: Repair the pipe to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the pipe, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Wall and floor around hand sink in patient tray line room noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Excellent prevention of cross contamination between dirty and clean dishes exhibited by dish staff. Proper glove usage and hand washing observed. Work orders have been put in for all physical facility issues noted in previous inspections. All food handler cards up to date. Discussed food manager recertification, managers will most likely retest through ServSafe.

Reviewed all paperwork with new Director of Food & Nutrition Services, Jennifer Small. Form 1-B, food handler and manager schedules given. Reviewed cooling procedure with executive chef. All logs well maintained, proper hand washing and glove usage observed. Recommended for permit renewal. Permit issued.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the wall around triple sink had accumulations of mold and grime.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Wall and floor in patient tray room and floor under steam kettles are not maintained in good repair. Floor and wall tiles missing and leak observed coming from ceiling.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Several facility issues observed, floors and walls, PIC has work orders in for repairs to be done and is working with DePaul. All food service practices good, all temperature logs maintained.
  • Hair Restraints - Effectiveness
    Observation: Employees observed working in the food service area without proper facial hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: wall near corner of triple sink
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Equipment and Utensils, Air-Drying Required (corrected on site)
    Observation: Large containers were found stacked wet after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.

Observed a few date labels with expired dates but items are dated for less time than required by FDA Food Code, so they are still good. Spoke with Mr. Mason and Ms. Rivers about making sure dates are checked on items in stock. Observed temperatures actively being taken during various stages of cooking process and temperature logs well maintained.
  • Warewashing Equipment, Determining Chemical Sanitizer Concentration (corrected on site)
    Observation: The chemical sanitizer was too weak. Sanitizer in triple-sink observed below 100ppm, made about 1 hour before. Remade and observed at 150ppm
    Correction: Obtain a quaternary ammonium chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
  • Physical Facilities in Good Repair
    Observation: Broken floor tiles in tray line room and under large kettles
    Correction: water observed leaking from ceiling tiles in tray line room, thus not maintained in good repair.

    Observation: The cutting boards (green and red) are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Indoor Areas - Surface Characteristics
    Observation: The indoor wall material located in tray line room (chair rail) does not meet the standard of: 1. smooth, durable and easily cleanable 2. closely woven and easily cleanable carpet 3. nonabsorbent.

  • Light Bulbs Protective Shielding
    Observation: Light bulb in heat lamp and regular lamps on employee line, and almost all bulb covers in dish room are cracked thus - not shielded, coated, or otherwise shatter-resistent.
    Correction: Shield or replace light bulb with a coated or shatter-resistent bulb.

Morrison's has recently taken over the DePaul kitchen. Michele Rivers is the new Head of Nutrition. Temperature logs maintained, excellent date marking observed. Proper hand washing techniques and glove usage exhibited by all staff. Recommended for new Health Dept permit. Permit application to be filled out and mailed in.

    Observation: The nonfood contact surface of the lower interior panel of tilt skillet is unsealed plywood and is not corrosion resistant, nonabsorbent, and/or smooth.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
