    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch in that inner panel of ice machine observed with heavy accumulation of black mildew.

    Observation: The employee observed drinking coffee from an uncovered container in the food preparation area in kitchen.
    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands, (2) The container, and (3) Exposed food, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.

    Observation: Employees observed handling ready-to-eat (RTE) salad with their bare hands.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking*
    Observation: The prepared ready-to-eat (RTE) chilli, gravy, beef gravy, beef chip, coleslaw and tuna salad in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: The inner panel of ice machine observed with black mildew accumulation.
    Correction: Operator voluntarily wiped the inner panel of said ice machine. Operator instructed to discard the top section of said ice.

    Observation: Food contact surfaces of the meat slicer used to prepare potentially hazardous food items were observed soiled with accumulations of food residues.
    Correction: Clean food-contact surfaces of meat slicer no less than every 4 hours to prevent the growth microorganisms on those surfaces.
  • Hand Drying Provision (corrected on site) (repeated violation)
    Observation: No disposable towels were provided at the hand washing lavatories in the kitchen and waiter station.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials. Operator voluntarily provided paper towels in all handwashing facilities.

Follow up inspection conducted for complaint case ID# 37139
Observed floor/area around outdoor trash container clean of trash and old food debris. Old grease observed around grease tank. Operator instructed to clean said area.
Complaint addressed.

Case ID # 37139
A complaint was received on 10/14/14 stating that the oil, food and trash from the diner gets very unhealthy at the outdoor trash container. Also, there is a storm drain nearby where the food grease is flowing into on rainy days. This is an attraction for rats and mice.
Operator was unaware of the complain. During the inspection some trash and grease with food were observed outside the container. Operator voluntarily placed said trash inside the container and cleaned the grease with hot water.
Area around the outdoor trash container must be cleaned. Trash shall always be discarded in the container and the containers must always be kept clean.
Follow up 10/21/14.

10/20/2014Complaint
Follow-up to verify correction of leak/drip in the walk in cooler. A new drain line has been installed in the exterior of the walk in. The seam that was previously leaking appeared to be dry. Please clean this area.

09/25/2014Follow-up
Leak/drip in walk in cooler still observed as active during inspection. Water was observed dripping onto packaged and uncovered food. Uncovered and partly covered food was removed from the cooler to be discarded by PIC. Repair as needed prior to next inspection. Next inspection Thursday September 25 2:00 PM.

Maintain the corrected items as corrected.
Leak/drip in walk in -> scheduled for repair later this week per PIC. Disposable containers -> some still observed in use -> continue to discard disposable food containers.
