Food suppliers are US Foods and Sam's Club.Observed good handwashing, glove use, and datemarking. Discussed employee health, form 1B is used. Discussed setting up the 3 compartment sink. Discussed cooling procedures and parameters. Discussed addressing leak at sanitizer dispenser. PIC to contact EHS when cleaning of physical facility has been addressed.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: The following areas noted in need of cleaning: under/along the fryer, floor along the cook line and around reach in coolers, wall and underneath 3 comparmtment sink, under/wall behind the ice machine.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Discussed cooling of foods including nacho cheese, chicken/chicken salad and soup (only during cold months). BBQ is commercial product and reheated when ordered. Hamburgers are not undercooked and no consumer advisory is needed. Discussed employee health. Employee health information posted. No new menu items. Orders for food are taken in both the smoking and nonsmoking areas and delivered to customers in the nonsmoking area. Main suppliers are US Foods, Sam's Club and Restaurant Depot.
  • Cloths - Wiping Cloths - Use Limitation (corrected on site)
    Observation: Observed wet wiping cloths not stored in sanitizer. Wiping cloths improperly stored between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Lighting, Intensity
    Observation: Less than 50 foot candles of light was observed under the hood system. Lights were not working
    Correction: Recommend replacing bulbs or repairing fixtures. Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.

Observed hand washing by employees. Employee health information posted including OEHS Guide to Employee Health and Form 1B signed by employees. PIC requested new copies of guide and Form 1B. New detergent and sanitizer dispensing system installed at 3 basin sink. Recommend cleaning in the following areas: hood filters, floor behind ice machine, exterior surface of grill and fryer, floor under grill and fryer, wall behind grill and fryer, bottom shelf of prep table next to fryer. Discussed cooling process for soups.
No violation noted during this evaluation.

All areas of concern listed on risk inspection on 11/6/12 have been addressed. Observed employees washing hands. Employee health guidelines and Form 1B for all employees are posted. Chicken and hamburgers are raw meat. Sausage is precooked. Serving times 9-10:30 for breakfast items
