Updated service/commissary agreement received. Employee Health form 1B sigend. Fills freshwater with white food grade hose directly into 35 gallon tank. Grey water is dumped every 2 days. Takes whole 50 gallon container out. Rinse hose to grey water needs securring.

Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site) Observation: The prepared ready-to-eat (RTE) garlic sauce, prepped veggies in the refrigeration unit is not properly dated for disposition.
Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
Equipment and Utensils - Durability Observation: The equipment is not designed and constructed to be durable.(food stored prepped in on food grade containers)
Correction: Remove this item from the facility and replace with an approved unit as needed. Equipment and utensils must be capable of maintaining their original characteristics in order to be easily cleaned and to prevent contamination of food.

Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Ashland Strawberry Faire. Menu: Chicken wings that are recieved pre-cooked and frozen. Food supplier is Restaurant Depot. Discussed the following: 1. Employee Health, EHS to provide Form 1B and OEHS Guidelines for use. 2. Handwashing and glove use. 3. Setting up the 3 compartment sink, steramine concentration shall remain between 200-400ppm. 4. No cooling in facility. No violation noted during this evaluation.

Risk Factor
Items identified at pre-opening inspection on 4/20 have been corrected. Owner stated that new gaskets for reach in cooler have been ordered. Hanover commissary is not permitted at this time. EHSS and owner discussed options presented to owner in email on 4/21/15. Owner stated that he is unable to find a commissary in Hanover at this time. Other options include locating a commissary in another county that is permitted and working with the local health department to permit the unit or being a self-contained unit and permitted by the local health department in jurisdiction where owner lives. No violation noted during this evaluation.
