Observation: The milk box on line A does not appear to be holding at the proper temperature. Ambient air temperature read 46'F. Milk that was just placed in milkbox was relocated to WIC.
Correction: Repair the milkbox to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the milkbox, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

3-compartment sink - 200 ppm quaternary ammonium Facility is very clean. Thank you. Discussed: Ice accumulation in WIF. Until repairs have been made, do not store boxes of food on shelves where leak is occurring. Abbreviations: WIC - walk in cooler

Observation: The prepared ready-to-eat (RTE) chicken salad in the refrigeration unit was not discarded by the ""consume by"" date. Chicken salad was prepared on 5/22 and should have been discarded 1 day ago. PIC discarded food.
Correction: Discard the food at this time and ensure all prepared RTE food is served, sold or discarded by the ""consume by"" date. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

Observation: WIF was observed in a state of disrepair and damaged. Heavy ice build-up observed in WIF and shelves. Work order has been placed.
Correction: Repair the equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the equipment replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: The handwashing facility located in the ala carte/salad room is blocked by the wiping cloth bucket, preventing access by employees for easy handwashing. PIC removed bucket.
Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the equipment preventing its use.
