In the event of a change in operator or remodeling, the facility must be brought up to the current code requirements. This may include the addition of one or more handsinks, improved lighting, repair of structural deficiencies, and repair or replacement of damaged equipment, and proper installation of laundry facilities. Note: Walk-in cooler condenser was being worked on by maintenance during the time of the inspections. A pan of pasta dated 9-12-2014 was in the walk-in cooler. EHS brought it the attention of the PIC, and she stated that the dish was being thawed out and it had just been pulled out of the freezer. The pasta will be used 10-02-14

Observation: Thermometers were observed stored in a container of sanitizing solution on the preparation table.
Correction: Store thermometers in a dry and clean container without sanitizer.

Observation: The following hysical structures is not maintained in good repair:
>> Holes in walls located behind the mechanical dishwasher.
>> Holes in walls at kitchen handsink.
>> Storage room floors surface observed with cracks.
>> Storage room ceiling adjacent to teh manager's desk observed with rusty/ water damage.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: Toilet room door in the kitchen are being kept open
Correction: Keep toilet room doors closed except when cleaning or during maintenance operations to prevent insect and rodent entrance and the associated potential for the spread of disease.

Observation: The nonfood contact surface of the shelving in the wash room is not corrosion resistant, nonabsorbent, and/or smooth. Shelving was observed made of wood.
Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent. Discontinue the use of any materials that are absorbent (i.e wood , cardboard). Wood may be painted to protect it from any spillage or splash.

Observation: Back door of the food establishment is not protected against entry of insects and rodents. Large gap observed at the bottom of the door
Correction: Protect the food establishment against the entry of insects and rodents by providing tight fitting doors.

Observation: The inside of the ice bin had an accumulation of mold.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues. School was closed for 6 days due to inclement weather, which gave time for the mold to grow.

Observation: The following was not maintained in good repair:
1. Inside of walk-in refrigerator around door/wall had cracks and was not sealed.
2. Floors under preparation sink was not sealed to the wall. Large gaps were present.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: Rodent bait found behind ice machine and reach-in refrigerator dispensed and spread over the floor.

A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
