    Observation: Observed the dumpster and the area under the dumpster filthy.
    Correction: Clean.
  • Light Bulbs Protective Shielding (repeated violation)
    Observation: Observed several lights without shields and several lights without end caps on the new shields.
    Correction: Properly shield lights.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Observed the floor buckling, missing tiles and/or low grout in the following areas: doorway into the kitchen from the dining area, the bar, the beer cooler,the beverage station and the walk-in cooler.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Floors to be done by 11/25/15 according to area director.

Floor has been an issue and cited on the 10/7/2015. The floor is very bad and most be corrected before the next permit is issued. Current permit expires on 10/31/2015.

    Observation: Observed the bar tender cut fresh strawberries and was not wearing gloves.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Observed torn gaskets on the low boy drawers.
    Correction: Replace.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: Observed old labels on the outside of the clean pans.
    Correction: Remove labels and the residue left by the labels.
  • Critical: Sewage - Conveying Sewage*
    Observation: Observed the 3 compartment sink in the bar area draining directly onto the floor. Also, all the drain lines for the ice bins were draining directly onto the floor.
    Correction: These drains should be going into the floor drains that are provided.

The floor in the bar area as well as in the kitchen (expo line and dish) is in very poor condition and needs to be repaired by 10/31/15 when the current permit expires. Also, several sets of lights were not shielded.

    Observation: Observed the TCS foods (salmon, shrimp, steaks, ribs, etc.) in the low boy at 60°F.
    Correction: Product was discarded by management. The unit was not properly plugged in to the socket.
  • Equipment - Non-Food Contact Surfaces and Utensils (corrected on site)
    Observation: Observed a build-up in the nozzle of the ice tea dispenser.
    Correction: Taken apart to be cleaned.

No risk factor violations observed. Gloves worn, utensils used. Discussed set up of dishmachine and check supplies (soap), drain pipe from dipper well should be directed to a cleanable drain. Check under shelves and in corners for floor debris. Dishmachine - 187 degrees at 17 psi final rinse. 3 compartment sink - Quat sanitizer - 200 ppm - test kit (ok). Suggest change out trash dumpster prior to hot weather.

    Observation: Found several cold unit gaskets that are damaged.
    Correction: All kitchen equipment is to be in good condition. Please replace damaged gaskets.

Gloves worn. Discussed damaged floor at cook line which needs to be repaired. Wall section needs repair at office door and refrigeration door gaskets at the bar all need repair. All items need to be repaired for issuance of next permit due in October. Prep table shelf no longer cleanable (rust). Dishmachine 190 degrees at 20 psi final rinse.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Found chipotle chicken at 52 degrees on the rail.
    Correction: All cold hold to be 41 degrees or less. All product was iced down and tech came for repair (low on coolant).

    Observation: Found large plastic lexan container of pasta and noodles in walk-in at 81 degrees. Procedure was not followed for cooling pasta product.
    Correction: Product to be cooled to below 70 degrees within 2 hours, then 41 degrees in next four. Product was pulled and cooled to 39.7 degrees.
