5-202.14 - --In the janitor's closet by the dining area, the mop sink faucet has a built in backflow prevention device (atmospheric vacuum breaker). This device has failed and it leaks when the water is turned on. A hose used for filling mop buckets is hooked to the faucet (and is below the flood rim of the sink). This was cited during the most recent inspection and has not been fixed. The person in charge was instructed to leave the hose off at all times until this device was repaired or replaced. KDA_4_20211A2 - --Two plastic food containers and one are cracked and one plastic measuring cup is chipped and broken. This can prevent proper cleaning and pieces of plastic can end up in foods (discarded). KDA_4_60111A - --One plastic food container lid stored as clean had adhesive on the top of the lid (removed for cleaning) One single use plastic food container lid intended for re-use had a sticker on top of the lid (removed for cleaning). KDA_4_60111C - --Mold is on the door and lid gaskets on both milk reach in coolers. This one item was less than 10 percent of items required to be date marked. KDA_4_20211A2 - --One plastic measuring pitcher and one plastic ice scoop are cracked, this prevents proper cleaning and pieces of plastic can end up in food (discarded). KDA_4_60111C - --Dust is built up in several spots in the vent hood above the dishwasher. KDA_4_60211E4 - --(b) - In the ice machine bin, a small amount of mold is on the stainless steel strip and white plastic piece above the ice. KDA_4_90411B - --In a plastic self serve container, single use plastic spoons are stored turned different directions. KDA_5_20515B - --On the cold water line that services the dishwasher pre-wash sink, under the table, the handle for the shut off valve is missing and the cold water is turned off. KDA_7_20111B - --In the kitchen were unwashed cucumbers, onions, and tomatoes that were just brought over from the main kitchen (taken straight from a box as per a cook). The only place to wash produce is in the 3-vat sink, which has a chemical dispensing unit above it (produce removed and taken back to main kitchen). This has been cited before and discussed with the food service manager several times, including last on 27 October 2015. Produce must be washed at the main kitchen unless the dispensing unit is removed, or a separate produce prep sink is installed.
