Located on the kitchen counter, there was an unlabeled container of brown powder and an unlabeled container of white powder which the PIC identified as yeast and salt. COS- labeled. KDA_3_30511A1 - --Located in the WIC, there was a box of potatoes that was stored on the floor. KDA_3_50118A2 - --Located in the RIC under the MT, there was a container of cut lettuce with no date mark. Person in charge (PIC) indicated that it had been cut 9/19. COS- dated. Located in the Master Bilt RIC in the wait area, there was an open container of milk with no date. PIC indicated that it had been opened on 9/19. COS- dated. KDA_4_60111C - --There is an accumulation of grease and food debris in the oven and on the stove in the kitchen. KDA_4_60211E4 - --There is mold on the inside rim of the ice machine. KDA_4_90311A - --Located in the dry storage room, there was a box of gloves that was stored on the floor. KDA_4_90312A - --There are storage shelves with single use paper products and wiping cloths in the employee restroom. KDA_4_90411B - --Located on a rack next to the MT, there were cups of clean utensils with handles inside the cups and not inverted. KDA_5_50117 - --There is no covered trash can in either the employee restroom or the unisex public restroom. KDA_6_50112A - --There is an accumulation of dust and debris on the floor of the storage rooms and kitchen areas.
