- Observed an employee only washing their hands approximately 8 seconds. Employee was retrained by inspector and rewashed his hands correctly. Manager must retrain all staff on proper hand washing technique and send documentation to TCHD, Attn: Kim by 2/11/16.
- The shelves in the walk in cooler and utensils stores in a container on the clean dish shelf had a build up of debris. Wash, rinse, and sanitize them.
- Repair the leaky faucets at the bread room prep sink and the three compartment sink by the dish machine.
- Replace broken and missing tiles as discussed with owner.
- The soap and paper towel dispensers attached to the wall in the bread room and walls in the prep kitchen had a build up of debris Clean them.

- Observed leaky handwashing sink in kitchen with a bucket below collecting drip water. Work order has been placed for repair per Mike. Found clogged toilet in women's restroom. Repair.

- Found mop stored in mop cart. Ensure mop is hung to air dry when not in use.

In the sandwich prep cooler the container for the ham has a crack in it. The racks in the walk in cooler are rusted.
- There is buildup in the can opener in the bread room. Also food debris on shelving throughout the facility.

- Found build-up on walls especially in the bread room.
- Found stained/damaged ceiling tiles especially in the pizza prep room.
- The light shield in the pizza prep room is damaged and cracked.
