- The soda lines had a build up of debris under the backside of the front counter. Clean them.
- Observed several employees without hair restraints. Hair restraints must be worn by all employees.
- Repaint the rusty outside of the Blodgett oven.
- The insides of coolers and freezers and front counter cup holder had a build up of debris. Clean and sanitize these areas.
- The fryers, fry station, burger conveyor, and grill had a build up of debris. Clean and sanitize these areas.
- The cooler door seals, cooler and freezer door handles, wheels on the cooking equipment, outside of coolers, outside of Blodgett ovens, under the hot serving line, and the mop sink had a build up of debris. Clean and sanitize these areas.
- Remove the cardboard from under the fryers and drive thru area. Re-grout floors in areas discussed with manager.
- Found build-up of debris on floors under and around equipment. Clean the floors.
- Repair damaged wall on the east side of the mop room.
- The walls had a build up of debris in the kitchen. Clean the walls.

- Found container of ground meat in walk in cooler at 79 F. There was no label or cool down documentation. Potentially hazardous food requiring refrigeration after preparation shall be labeled or tagged with the date and time of preparation and rapidly cooled to an internal temperature of 41°F. Ground meat was voluntarily disposed of by manager at time of inspection. Emailed information to facility on cool down porocedures along with inspection report.
- Observed two employees washing their hands and failing to use a paper towel to turn off faucet. Provide proper handwashing training to all employees and submit documentation by 12/29/14.
- Found an accumulation of debris on the following items and surfaces: Hansink, Metal rack by freezer, Cart in walk in cooler, All stand alone equipment including cooling units, fryers, and grills, Cup dispenser at soda dispenser machine near register, Spill well at soda machine in dining room. Wash, rinse, and sanitize these items and surfaces and ensure routine cleaning.
- Floors in areas of the facility had an accumulation of debris and grease. There was a large grease spill in utility room. Effectively clean floors and include in routine cleaning.
- Walls in facility had an accumulation of debris. Noticed a lot of debris buildup in utility room and near the grill.
- Observed dust above the vents near the handsink. Clean and ensure routine cleaning.
