Observed an employee wash hands without soap. Described to the manager proper hand washing and gave the new hand washing hand out.
Observed food service equipment stored on the floor in the crawl-space storage. Food service equipment must be stored a minimum of 6 inches off the floor. Per the kitchen manager everything in the crawl space is not in use and the crawl space will be walled off.

Observed a gallon of open milk that was not labeled after opening and had a use by date of 9/29/15. The milk was voluntarily discarded by the manager during this inspection.
Observed 4 bags of frozen chicken wings in the freezer that were stored in zip loc bags. These chicken wings appeared to have been a pre-cooked item that was then frozen. The chicken wings were not labeled. This facility is a category 2 facility and is not permitted to cool down food items for re-service. The 4 bags of chicken wings were voluntarily discarded at the time of this inspection.
Observed debris accumulation on the following items/surfaces: The stove located to the right of the flat top grill, The utensil drawer next to the stove, Cabinets throughout kitchen as discussed with manager during this inspection, Both refrigerators in the kitchen. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
