- Found temp of large container of salsa 52* in front waitress station. Food Mgr discarded salsa.
- Three thermometers were inaccurate.
- Found aluminane foil to cover bottom shelf inside lower reachin cooler.
- Found food scum/rust buildup on metal shelves inside one walk-in cooler.
- Back floor drain does not drain properly.
- Found some small screw holes in walls in dishwash area.
- Found wet mop stored against wall in front waitress area.

- Raw chicken next to the grill was 52f. There was too much chicken in the container for the ice bath to keep cool. The chicken was moved into the walk-in cooler during the inspection. I told the employee to only fill the container half full and refill when needed. This was something I had brought up during the last inspection.
- Observed a container of cheese dip in the walk in cooler labeled 9/4/15. The cheese was voluntarily dicarded. Per employee it was mislabeled.
- The sanitizer bucket below the spices is 10ppm. Discussed making sure employees test every time and use measuring devices so it is made the same way every time. All of the sanitizer buckets were different concentrations.
- The hand sink by the prep sink has a faucet with 1 handle. When I pushed it to the left for hot water at first it didn't work, then later hot water would shoot out randomly. This sink doesn't get used very often.
- Observed the back door not entirely closed. It was closed by an employee during the inspection.
- Observed a bottle of steel cleaner on the same shelf as tortilla and other food contact surfaces. The bottle was moved during the inspection. There was an unlabeled bottle of chemicals by the dish washing machine. The bottle was labeled during the inspection.

- There were foods uncovered throughout the facility. In the walk in cooler with tortillas there were cut onions that had plastic wrap on top but they weren't covered properly, under the prep cooler there were sauces, jalapenos and other foods, in the reach in freezer chiles, in the reach in cooler salsa.
- There was an unlabeled personal container in the beer cooler. It was removed.
- There is tape on the light switch to the dry storage area. In the dry storage area there is still wood on the side of the shelves. There is a board under the can rack.
- Found no chlorine sanitizer in dish machine. It was run several times and tested by the owner who agreed it was 0ppm. The owner and employee attempted to prime the sanitizer but it was still 0ppm. Per owner he tested it yesterday morning and it was 50ppm. He called a repair man during inspection. Maintain at 50-100 ppm. Dishes must be sanitized in 3-comparment sink until corrected. The repair man came at the end of the inspection and proceeded to work on the dish machine.

- The bottom of the beer cooler in the bar area it appears as if something has spilled. The handle to the walk in cooler is very bumpy as if there is an accumulation of food buildup. At the start of the inspection the can opener had buildup in it. It was removed and cleaned during the inspection. The hand sink across from the dish machine has gum on the side of it. The cart has food debris on it.
- There are cups stacked together wet.

- There was Cascade stored on a shelf in the dish machine area over the area where clean dishes come out. The Cascade was moved to the bottom shelf.

- Observed in the prep cooler, raw chorizo stored over open salsa. The chorizo was moved to the bottom and covered.
- In the reach in prep cooler there were multiple foods left uncovered.
- Observed cooks leaving gloves on even when done with food prep. One cook was holding a pot holder the entire time, the other had a rag on his apron which he touched. The rag was later removed. The manager spoke to employees.
- Observed exposed/unsealed wood throughout facility. Broken seals on coolers.
- Observed water dripping under 3-compartment sink and collecting on the floor. Also there is an open pipe underneath.
- The door by the walk in cooler wasn't closed all the way.
