ADPH 420-3-22.02(3)(a) Provide at least one designated person with certification from an accredited program within the past 5 years, or verification of attendance of an accredited program within the past 12 months. / Managers certification expired August 18, 2011.
3-501.16 Maintain potentially hazardous food at 41 F or below. / 4 large covered plastic containers with raw chicken ranging from 44.4-50.3 F. Chicken has been in RI cooler this early this morning. COS-Manager discarded all chicken.
3-501.15 Use shallow pans that facilitate heat transfer when cooling food. / Raw chicken fingers 43 F cooling in covered plastic tubs inside the RI cooler. Chicken uncovered and moved to top shelf.
4-502.11 Calibrate thermometers according to manufacturer specifications as often as necessary to ensure accuracy. / One food thermometer reading 20 F at room temperature. Manager discarded this thermometer. Establishment will use another food thermometer that they have on site.
4-501.116 Provide test strips with color chart for quaternary ammonia sanitizer.

7-202.12 Use only pesticides labeled for use in a food establishment. / Two spray cans of Raid stored above mop service sink in kitchen, and one can of Bengal Pest spray located on floor beside mop service sink in kitchen. COS- Manager removed Raid and Bengal from facility.
4-602.11 Clean fan covers in WI cooler. / WI cooler fan covers and ceiling above fan covers with dust accumulation. WI freezer ceiling beside WI fans with heavy ice accumulation.
4-501.11(B), 4-201.11, 4-202.16 Replace damaged gaskets. / A couple of RI cooler door gaskets torn.
