MUST SCRAPE AND REPAINT PEELING PAINT ON HOOD OVER COOKING EQUIPMENT IN DELI HOT FOOD PREP AREA. MUST REPLACE WORN STRIPPING ON THE LEFT SIDES OF DELI WALK IN COOLER AND MEAT PREP COOLERS. MUST REPLACE BROKEN SLIDING DOOR TO DISPLAY COOLER IN FRESH MEAT SECTION. MUST ALSO REPLACE MISSING SLIDING DOORS IN DELI AND FRESH MEAT SECTIONS.

MUST MAINTAIN POTENTIALLY HAZARDOUS COLD FOODS AT 40 DEGREES F AND BELOW. CRITICAL CITATION ISSUED 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES AND HOUSE FLIES INSIDE OF THE STORE. OBSERVED APPROXIMATELY 40 LIVE FRUIT FLIES ON THE WALLS, CEILING TILES AND BREAD RACKS IN THE BAKERY AREA AND ON THE WALLS AND CEILING IN THE DELI AREA. ALSO OBSERVED 5 HOUSE FLIES FLYING THROUGHOUT THE STORE. TECHNICIAN FROM MARK'S PEST CONTROL WAS ONSITE DURING THE INSPECTION. OBSERVED 2 FLY STRIPS HUNG FROM THE CEILING IN THE DELI PREP AREA. MANAGEMENT REMOVED THE FLY STRIPS UPON REQUEST. CLARK STREET RECEIVING DOOR IS NOT RODENT PROOF. OBSERVED A 1/2' GAP ALONG THE BOTTOM OF THE DOOR. MUST RODENT PROOF THE DOOR. SERIOUS VIOLATION 7-38-030.
