All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REMOVE THE CARDBOARD USED TO LINE THE SHELVES AND FLOOR OF THE WALK-IN COOLER. REPAIR THE DOOR HANDLE FOR THE WALK-IN FREEZER.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE WIRE RACKS IN THE WALK-IN COOLER.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN WALLS OF THE PREP AND STORAGE AREAS. REPLACE THE MISSING WALL TILES FROM THE AREA NEAR THE SIDE DOOR. DETAIL CLEAN THE CEILING OF THE PREP AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR IN THE PREP AREA AND STORAGE AREAS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. OBSERVED STAGNANT WATER AND JUICES IN THE OVERFLOW DRAIN PAN FOR THE JUICE DISPENSER. IT MUST BE SLOPED TO DRAIN. CLEAN AND SANITIZE TO ELIMINATE PEST ACTIVITY.

ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

MUST USE A FOOD GRADE STORAGE BAG ONLY.
