Facilities not maintaining proper temperature while storing potentially hazardous foods. Found two coolers in deli area at improper temperature. Salad cooler at 46.2 F to 53.7 F and Sausage cooler at 52.1 F. Instructed that coolers must manitain 40 F or below. 'Held for Inspection' tag placed on coolers. Critical citation issued 7-38-005(A).
Potentially hazardous foods does not meetr temperature requirement during storage, preparation, display and service. Found approximately 36-6oz yogurt at the bottom of sausage cooler 52.3 F. Salad cooler found Tuna Salad approximately 8lbs at 61.5F, pork meat patties at 47.3 F. Instructed manager that potentially hazardous foods must maintain 40 F or below. Manager voluntarily denatured and discarded. Value $100. Critical citation issued 7-38-005(A).
Storage shelves, bulk storage containers, cutting boards, prep table, prep counter, sinks, cooking equipment and fan covers inside walk-in not cleaned. Instructed to detail clean and sanitize.
Floors in prep area, storage area, basement and around and under equipment not cleaned. Instructed to detail clean and reseal floor in basement.
Ceiling tile and light covers not maintained. Instructed to replace any worn ceiling tile and detail clean light covers.
Thermometere not provided inside walk-in cooler. Instructed to provided hanging thermometer visible inside all coolers.
Unnecessary articles stored in basement. Instructed to remove all unused articles and detail clean area, elevate stock/supplies 6 inches or higher and away from wall.
Foods stored inside coolers not labeled or dated. Instructed to labeled and dated all foods not in original containers.

Found/observed rodent droppings, (10 ) in rear kitchen area, and(40 )rodent droppings in bsmt storage area under pallets, Citation issued.
Found/observed food items not properly dated/labeled in walk in cooler, walk in freezer, and reach in coolers, must date and label food items in walk in cooler, walk in freezer, and reach in coolers.
Rear storage shelving that had peeling paint, shall be repaired/sealed, or replaced.
Non food contact surfaces of walk in cooler shelving/racks not clean need detailed cleaning(crevices), non food contact surfaces of cooler shelving, prep tables(lower shelving)not clean need detailed cleaning, rear storage shelving/racks of prep area need detailed cleaning(crevices), bsmt storage shelving need cleaning(detailed).
Floors under heavy equipment, prep tables in prep areas need cleaning, floors in bsmt storage areas need detailed cleaning(corners), also must sanitize area were rodent droppings were found.
Missing, damaged ceiling tiles in storage areas shall be replaced, exposed insulation, gaps on ceiling in rear prep area shall be repaired/sealed.
Missing light shields in bsmt storage areas, shall be provided, light shields in prep areas need cleaning.
Exhaust vents(ventilation) in walk in cooler, coolers, and prep areas need cleaning.
